老年人喉镜观察及嗓音声学和空气动力学分析

摘　　要：目的探讨老年人喉部形态特点及嗓音声学和空气动力学特征。方法对老年组(95例,≥65岁)和中青年组(102例,25-45岁)的正常人进行喉镜检查及嗓音声学、空气动力学检测,比较声门闭合状态与嗓音参数。结果老年组喉镜检查主要表现为声带萎缩及水肿、声门闭合不全、室带肥厚、声门偏斜。与中青年组比较,老年组男性基频(F0)升高,而女性F0降低,老年组男女性基频微扰和振幅微扰均增高,发音障碍指数(DSI)降低,最长发声时间(MPT)缩短,平均气流率(MFR)增高(P<0.05)。老年组声门闭合不全者MFR显著高于声门完全闭合者(P<0.01)。结论正常老年人喉部具有特征性形态改变,嗓音存在声学、空气动力学参数的异常。Objective To investigate the characteristics of laryngeal morphology,voice acoustics and aerodynamics in healtly elderly.Methods Laryngoscopic observation and voice acoustic and aerodynamic analysis were performed in 95 elder people aged over 65 years old（group A） and 102 persons aged from 25 to 45 years old（group B）.The relationship between the status of glottic closure and voice parameters was compared.Results The laryngoscopy showed vocal fold atrophy and edema,glottic incompetence,hypertrophic ventricular fold,and glottic deviation in group A.Compared with group B,fundamental frequency（F0） in the males was elevated,while that in the females was declined in group A.The jitter and shimmer were increased,but dysphonia severity index（DSI） and maximum phonation time（MPT） were decreased,and mean flow rate（MFR） was increased（P0.05）.The MFR in group A with glottic incompetence was significantly higher than that with glottic completely closed（P0.01）.ConclusionThe larynx of elder people has morphologically characteristic changes with some abnormalities in voice acoustics and aerodynamics.
