超声乳化人工晶体植入联合房角分离术与小梁切除术治疗闭角型青光眼的临床效果比较

摘　　要：目的对超声乳化与小梁切除术治疗闭角型青光眼的临床效果进行比较分析。方法 58例闭角型青光眼患者中,应用超声乳化人工晶体植入联合房角分离术方法治疗的患者设立为A组,共32例,其余应用小梁切除术治疗的患者设立为B组,共26例,比较两组患者术前及术后3个月视力恢复情况及眼压变化情况。结果术前A组和B组患者的平均视力差异不显著,术后3个月复查,A组和B组的平均视力均较术前明显改善,且A组术后3个月的平均视力较B组改善更明显,差异有统计学意义(P<0.05)。术前A组和B组患者的眼压差异不显著,术后3个月复查,A组和B组的眼压均较术前明显降低(P<0.01),且A组术后3个月的眼压较B组降低更显著,差异有统计学意义(P<0.05)。结论与传统小梁切除术相比较,超声乳化人工晶体植入联合房角分离术治疗闭角型青光眼效果更好,能明显改善患者的视力,值得推广和应用。Objective To compare the clinical effect of phacoemulsification and trabeculectomy for angle-closureglaucoma. Methods 58 cases of patients with angle-closure glaucoma,the ultrasound emulsification methods oftreatment of patients with established as group A,a total of 32 cases,set up the rest of the application trabeculectomypatients（group B）,a total of 26 cases,the two groups were compared surgery before and 3 months after surgery visualacuity and intraocular pressure changes. Results The preoperative group A and group B patients mean visual acuitydifference is not significant,significant improvement in mean visual acuity compared with preoperative postoperative 3months later,group A and group B’and group A after 3 months,mean visual acuity more significant improvements thanin group B,the difference was statistically significant（P〈0.05）. Preoperative IOP of group A and group B patientsthe difference was not significant IOP compared with preoperative postoperative 3 months later, group A and group Bwas significantly lower（P 〈 0.01） in group A after 3 months the IOP reduced more significantly than in group B,thedifference was statistically significant（P 〈 0.05）. Conclusion Compared with traditional trabeculectomy,phacoemulsification treatment of angle-closure glaucoma better, can significantly improve the patient＇s vision, worthy of promotionand application.
