摘 要:目的:评价丙氨酸氨基转移酶(alanine aminotransferase,ALT)作为血液质量判断指标的价值。方法:对经过初筛合格且ALT>40U/L的标本使用ELISA方法检测HBsAg、抗-HCV,利用核酸扩增检测技术(nucleic Acid amplification tech-niques,NAT)对标本进行HBV-DNA和HCV-RNA的检测。结果:3 675份无偿献血者标本中有397份(10.80%)ALT>40U/L。在这397份标本中有389份(97.99%)的HBsAg和抗-HCV定性均阴性、HBV-DNA和HCV-RNA定量均呈现低拷贝数。而另8份(2.01%)标本的HBV-DNA或HCV-RNA呈现高拷贝数,其中3份标本HBsAg定性阳性、HBV-DNA呈现高拷贝数;4份标本抗-HCV定性阳性、HCV-RNA呈现高拷贝数;1份标本HBV-DNA呈现高拷贝数,HBsAg和抗-HCV定性阴性。结论:ALT作为判断血液质量的指标将造成大量血液资源的浪费。采用NAT作为判断血液质量指标,既能保证血液质量,又能保证血液不浪费。Objective:Evaluate ALT as judgment index of blood quality. Methods: All of the samples are qualified in preliminary screening and meet the requirement of ALT 〉 40U/L. ELISA were performed to test HBsAg, anti-HCV. NAT(nucleic acid amplifica- tion techniques) were used to detect HBV-DNA and HCV-RNA. Results:Among 3 675 voluntary blood donors, 397 (10. 80% ) sam- ples showed ALT 〉 40U/L. Among the 397 samples, 389 (97.99%) were of negative HBsAg and anti-HCV. The quantification of HBV-DNA and HCV-RNA showed low copy number. However, 8(2.01% ) samples' HBV-DbIA or HCV-RNA appeared high concen- tration. Three of them were of positive HBsAg and HBV-DNA presented high copy number. And 4 samples were anti-HCV positive and HCV-RNA presented high concentration. One sample's HBV-DNA was of high concentration, but HBsAg and anti-HCV were nega- tive. Conclusion:Using ALT as judgment index of blood quality will waste a large number of blood resources. Using NAT as judgment index of blood quality can ensure the quality of blood and reduce the waste of blood resources.
