摘 要:目的探讨甲状腺功能减退症(甲减)患者糖化血红蛋白(HbA1c)水平的变化及其影响因素。方法调取我院健康体检人群资料,随机抽取甲减患者42例(甲减组),健康对照者56例(对照组),分别记录空腹血糖(FBG)、餐后2小时血糖(PBG)、血红蛋白(Hb)、HbA1c、促甲状腺素(TSH)、游离三碘甲状腺原氨酸(FT3)和游离甲状腺素(FT4)等资料,并作相关统计学分析。结果甲减组的PBG及HbA1c显著高于对照组,(5.62±1.82)mmol/L vs(5.34±1.33)mmol/L,(5.70±0.43)%vs(5.45±0.35)%,而FBG、Hb显著低于对照组,(4.61±0.48)mmol/L vs(4.92±0.64)mmol/L,(135.2±15.8)g/L vs(141.8±17.6)g/L(P<0.05或<0.01)。相关分析表明HbA1c与PBG呈显著正相关,与Hb呈显著负相关(P<0.05)。结论甲减能够升高HbA1c水平,在使用HbA1c评估血糖控制水平时应注意甲状腺激素的影响。Objective To investigate the effects of thyroid hormone on serum glycated hemoglobin levels in patients with hypothyroidism.Methods Forty-two patients with hypothyroidism and 56 healthy controls were randomly enrolled in the study.The fasting glucose(FBG),postprandial glucose(PBG),hemoglobin(Hb),glycated hemoglobin(HbA1c),thyroid stimulating hormone(TSH),free triiodothyronine(FT3) and free thyroxine(FT4) levels in the subjects were examined,then statistical analysis was performed.Results PBG and HbA1c levels in patients with hypothyroidism were significantly higher compared to those of healthy controls,(5.62±1.82) mmol/L vs(5.34±1.33) mmol/L,(5.70±0.43)% vs(5.45±0.35)%,while FBG and Hb levels in patients with hypothyroidism were significantly lower,(4.61±0.48) mmol/L vs(4.92±0.64) mmol/L,(135.2±15.8) g/L vs(141.8±17.6) g/L(P0.05 or 0.01).HbA1c was positively correlated with PBG and negatively correlated with Hb(P0.05).Conclusion Serum HbA1c was elevated in patients with hypothyroidism.The effects of thyroid hormone on HbA1c must be considered in interpreting HbA1c as an indicator of glycemic control.
