塞来昔布联合曲马多对膝关节置换患者早期康复的影响

摘　　要：目的观察塞来昔布联合曲马多干预疼痛在全膝置换患者术后早期康复中的作用。方法选择90例行单侧全膝关节置换的患者,随机分为3组。A组应用塞来昔布干预疼痛,B组应用曲马多干预疼痛,C组应用塞来昔布联合曲马多干预疼痛。三组术后均采取康复锻炼。观察术后不同时间点的静息痛及运动痛、不同时间点的术后膝关节主动活动度以及不良反应。结果C组、B组静息痛、运动痛评分基本相近,两组比较差异无统计学意义;C组静息痛、运动痛评分均比A组低,两组比较差异有统计学意义。C组、B组膝关节活动度基本相近,两组比较差异无统计学意义;C组膝关节活动度比A组高,两组比较差异有统计学意义。C组、A组与药物相关的不良反应较B组少。结论塞来昔布联合曲马多干预全膝置换患者的疼痛对早期康复有良好的效果,可促进膝关节功能的恢复。Objective To observe the effect of celecoxib and tramadol on early recovery after total knee arthroplasty.Methods 90 patients with unilateral total knee arthroplasty were enrolled and randomly assigned into three groups：celecoxib pain intervention was applied on group A;tramadol pain intervention was applied on group B;pain intervention with both celecoxib and tramadol was applied on group C.Immediate postoperative recovery exercises were undertaken in three groups and both postoperative rest pain and movement pain were observed at different time periods,meanwhile,the active range of motion of the knees and the complications were documented.Results Similar pain scores were found regarding both rest pain and movement pain in group B and group C without significant difference.Scores of the rest pains and movement pains in group C were significantly lower than those in group A.Although no significant difference was observed with regard to active range of motion of the knee in group B and group C,the active range of motion in group C were significantly higher than that in group A.Moreover,less adverse effects related to medicine in group C and group A were found than those in group B.Conclusion Positive effect of celecoxib and tramadol on early recovery after total knee arthroplasty can promote the functional recovery of knee joints.
