不同剂量布托啡诺用于老年患者术后镇痛的临床研究

摘　　要：目的探讨不同剂量布托啡诺用于老年患者术后镇痛效果及合理用药剂量。方法选择100例老年患者,行自控静脉镇痛(PCIA),随机分为5组,每组20例。布托啡诺剂量分别为:0.06、0.08、0.10、0.12、0.15μg/mL。记录术后4、8、12、24、48 h疼痛评分、镇静程度、追加按压镇痛泵的例数及不良反应。结果Ⅲ～Ⅴ组的VAS评分明显降低,Ⅴ组Ramsay评分明显高于Ⅰ～Ⅲ组(P<0.05),Ⅰ～Ⅲ组恶心、呕吐发生率较Ⅳ～Ⅴ组低(P<0.05);Ⅴ组呼吸抑制2例,Ⅴ组嗜睡的发生率明显增高(P<0.05);Ⅰ组各个时间段按压镇痛泵的次数明显增多(P<0.05)。结论剂量10 mg的布托啡诺静脉PCIA用于老年患者可获得较好的镇痛效果,不良反应发生率较低。Objective To investigate the effects of different doses of butorphanol on postoperative analgesia in elderly patients and its reasonable dose.Methods 100 elderly patients undergone patient-controlled intravenous analgesia（PCIA）were randomly divided into 5 groups,20 cases in each group.The doses of butorphanol were 0.06,0.08,0.10,0.12,0.15 μg/mL respectively.Pain score,the degree of sedation,case number of analgesia pump to press and adverse reaction were recorded postoperative 4,8,12,24,48 h.Results VAS score in the Ⅲ~Ⅴ groups was significantly lower,Ramsay score in V group was significantly higher than that in Ⅰ~Ⅲ groups（P0.05）,incidence of nausea and vomit in Ⅰ~Ⅲ groups were lower than that in Ⅳ~Ⅴ groups（P0.05）;there was 2 cases respiratory depression in V group,somnolence incidence in V group was significantly increased（P0.05）;pressing analgesia pump frequency was significantly increased at each time in group I（P0.05）.Conclusion The dose of 10 mg butorphanol PCIA for elderly patients can obtain a good analgesic effect with low adverse reaction rate.
