摘  要:目的探讨垂体后叶素联合葡萄糖酸钙用于预防高危因素剖宫产产后出血的效果和安全性。方法选择我院2011年9月至2012年8月具有高危因素的剖宫产产妇150例,随机分为3组,每组50例。A组为传统单纯催产素组,B组为垂体后叶素联合催产素组,C组为葡萄糖酸钙、垂体后叶素联合催产素组。结果①三组血清钙水平比较差异无统计学意义,但总体均稍低于正常。②C组术中、产后2 h、产后24 h出血量均显著低于A组、B组,三组比较差异有统计学意义(P<0.05)。③A组、B组、C组发生产后出血例数分别为14例(28.0%)、8例(16.0%)、2例(4.0%),三组产后出血发生率比较差异有统计学意义(P<0.05)。④三组无明显不良反应。结论局部多点注射垂体后叶素联合葡萄糖酸钙预防高危因素剖宫产的产后出血疗效明显。Objective To discuss the efficacy and safety of pituitrin combined with calcium gluconate for prevention of postpartum hemorrhage on high risk factors of cesarean section.Methods 150 patients with high risk factors for cesarean section in our hospital from September 2011 to August 2012 were randomly divided into 3 groups(50 cases in each group):traditional simple oxytocin group(group A),posterior pituitary the blade element joint oxytocin group(group B),calcium gluconate,pituitrin joint oxytocin group(group C).Results ①No significant difference was observed in serum calcium levels between the 3 groups,but overall level were slightly lower than normal level.②The blood loss of intraoperative,postpartum 2 h and postpartum 24 h in group C was significantly lower than those of group A and group B(P0.05).③There were 14 cases(28.0%),8 cases(16.0%)and 2 cases(4.0%)of postpartum hemorrhage in group A,group B and group C.There were significant differences between the 3 groups(P0.05).④No significant adverse effect was observed.Conclusion Local multipoint injection of pituitrin combined with calcium gluconate for prevention of high risk factors of cesarean section postpartum hemorrhage has better efficacy.
