摘 要:目的:探讨超声弹性成像技术评价2型糖尿病(T2DM)合并高血压(EH)患者动脉血管壁弹性的价值。方法:将65例T2DM患者根据是否合并高血压分为2组,T2DM组33例,T2DM合并EH组32例,同时选取正常对照组52例。分别应用超声弹性成像技术对右侧颈动脉、右侧肱动脉管壁弹性进行评估。结果:T2DM合并EH组和T2DM组颈动脉及肱动脉弹性比值明显高于对照组,差异具有统计学意义(均分别为P<0.001及P<0.05),同时T2DM合并EH组颈动脉及肱动脉弹性比值亦高于T2DM组,差异具有统计学意义(P<0.05)。T2DM合并EH组和T2DM组颈动脉内中膜厚度(IMT)均明显高于对照组,差异具有统计学意义(P<0.001)。颈动脉弹性比值与脉压、IMT正相关(r=0.254,P<0.01;r=0.287,P<0.01)。颈动脉IMT与脉压、血糖呈正相关(r=0.279,P<0.01;r=0.205,P<0.05)。肱动脉弹性比值与血糖正相关(r=0.375,P<0.01)。结论:超声弹性成像技术能方便、快捷、无创评价动脉血管壁弹性的早期变化。T2DM合并EH患者和T2DM患者颈动脉及肱动脉弹性均明显减低。Objective To assess the clinical value of elastosonography in estimating the arterial elasticity function in the patients with type 2 diabetes mellitus and hypertension.Methods A total of 117 cases were divided into three groups: type 2 diabetes mellitus group(T2DM,33 cases),type 2 diabetes mellitus with hypertension group(T2DM +EH,32 cases),and the control group(52 cases).The elasticity of right carotid and brachial arterial wall were evaluated by elastosonography.Results The strain ratios in right carotid and brachial arterial wall were significantly higher in the groups of T2DM+EH and T2DM than those of normal subjects(P0.001;P0.05,respectively).When compared with those in T2DM group,the strain ratios in right carotid and brachial arterial wall were significantly higher in T2DM +EH group(P 0.05).Intima-media thickness increased in the groups of T2DM +EH and T2DM than those of normal subjects(P 0.001).The strain ratio in carotid artery was correlated positively with pulse pressure and intima-media thickness(r=0.254,P0.01;r=0.287,P0.01).Intima-media thickness was correlated positively with pulse pressure and fasting blood glucose(r=0.279,P0.01;r=0.205,P0.05).The strain ratio in brachial artery was correlated positively with fasting blood glucose(r=0.375,P0.01).Conclusion The early change of the artery wall elasticity can be estimated conveniently,quickly and non-invasively by elastosonography.Arterial elasticity function is obviously lower in the patients with type 2 diabetes mellitus and type 2 diabetes mellitus with hypertension.
