浙江省松阳县并殖吸虫病疫源地现状调查

摘　　要：目的了解松阳县并殖吸虫病疫源地现状和人群流行情况,为并殖吸虫病的预防控制提供依据。方法选择3个山区乡(镇)3个村作为调查点,每点调查第二中间宿主溪蟹50～100只,采用捣碎沉淀法和心脏检测法分别检测溪蟹肌肉和心脏囊蚴感染情况;随机选取100～150人进行吃蟹方式调查,采集被调查者血清,ELISA检测并殖吸虫血清抗体。结果第二中间宿主溪蟹检出卫氏并殖吸虫和三平正并殖吸虫2种囊蚴,捣碎沉淀法检测154只溪蟹,肌肉中并殖吸虫囊蚴感染率为57.79%,心脏检测73只,囊蚴感染率为57.53%;人群血清并殖吸虫抗体检测379人,阳性17人,阳性率4.49%;人群吃蟹方式调查379人,133人有生食溪蟹习惯,生食率35.09%。结论松阳县属于卫氏并殖吸虫和三平正并殖吸虫混合流行区,应在山区人群中开展并殖吸虫病防治的健康教育工作,纠正生食或半生食溪蟹的不良生活习惯。Objective To ascertain the prevalence of paragonimiasis and affected populations in Songyang County in or- der to guide paragonimiasis prevention and control efforts. Methods Three villages of three mountain towns were se- lected as the survey sites, and 50 100 crabs from each site were examined as the second intermediate host of Paragonimus westerrnani. Metacercariae in crab muscle were detected by blending and precipitation and metacercariae in the heart were detected by examining the heart. A survey on eating raw crabs was conducted with a randomly selected sample of 100-150 people, and serum samples were collected to test for antibodies against paragonimiasis using an enzyme-linked immunosor- bent assay （ELISA）. Results Metacercariae of both P. westerrnani and Euparagorzirnus cenocopiosus were detected in crabs. The bodies of 154 crabs were blended and allowed to precipitate. Metacercariae were detected in muscle at a rate of 57.79% and in 73 hearts at a rate of 57.53%. Serum samples from 379 individuals were tested for antibodies against par- agonimiasis; these individuals tested positive at a rate of 4.49%. The survey of 379 people regarding the eating of raw crabs indicated that 35.09% ate raw crabs. Conclusion P. westerrnani and E. cenocopiosus are both responsible for outbreaks of paragonimiasis in Songyang County. Health education regarding paragonimiasis prevention should be expand- ed to the population in mountain areas to correct the habit of eating raw or undercooked crabs.
