心脏移植术规范化护理流程的探索

摘　　要：目的:探讨同种原位心脏移植术规范化护理流程和提高手术成功率、缩短手术时间的相关因素。方法:通过对23例患者实行双腔静脉原位心脏移植手术,分析总结心脏移植术中手术配合经验,征求心脏移植手术医师的意见和建议,查询国内外学术资料,整理出一整套手术配合规程,再通过临床实践检验,不断完善,以达到最快捷准确的手术配合效果,在一定程度上缩短手术时间,保证移植手术顺利完成。结果:本组患者无手术死亡。有3次同时为2例患者心脏移植,手术均顺利完成。术后住院时间7～30 d,ICU监护时间7～12 d。结论:规范化的护理流程对于心脏移植术顺利完成是有力的保障,护士的培训、严格的无菌管理、良好的心肌保护和医护配合是手术成功的重要因素。Objective：To investigate the standardized nursing process in orthotopic heart transplantation and relevant factors in improving the success rate of operation as well as shortening the operation time. Methods：Through analyzed the nursing experiences based on 23 cases of bicaval orthotopic heart trans- plantation, asked for surgeons＇s suggestion and inquired relevant academic materials,as a result to settle a set of surgery coordination procedures. Then ap- plied clinical practice test in order to get improvement,at last got the most fast and accurate surgery coordination effect,shorten the operation time to a certain extent and ensure the successful accomplishment of the surgery. Results：No one was dead. We even thrice successful finished two simultaneous cardiac trans- plantation operation. Their postoperative hospitalization time was 7 -30 days, ICU guardianship time was 7 - 12 days. Conclusion： Standardized nursing process is the powerful guarantee for the successful complishment of heart transplantation ,furthermore, nurse training, strict aseptic management, good myo- cardial protection and doctor - nurse cooperation are important factors for cardiac surgery＇s success.
