正常妊娠和妊高征孕妇血浆内皮素血栓素前列环素心钠素含量测定及意义

摘　　要：目的　了解妊娠期妇女血浆内皮素 (ET)、血栓素 (TXA2 )、前列环素 (PGI2 )、心钠素 (ANP)含量变化 ,探讨其与妊高征的关系。方法　采用放免法 ,在妊娠早、中、晚期及产后序贯测定血浆ET、TXA2 、PGI2 和ANP含量。正常妊娠142例 ,妊高征 36例 ,与非妊娠妇女 30例对照。结果　正常妊娠中期ET明显下降 (P <0 0 5 ) ,TXA2 、PGI2 、ANP含量与对照组无明显差异。妊娠晚期四项参数含量显著高于对照组 (P <0 0 1)。ET/ANP和TXA2 /PGI2 比值无显著变化。妊高征中期ET、TXA2 明显高于正常妊娠组 (P <0 0 1,P<0 0 5 )。ET/ANP ,TXA2 /PGI2 比值明显增高。妊高征妊娠晚期 ,除PGI2 外 ,其它三项参数及ET/ANP ,TXA2 /PGI2 比值均高于正常妊娠组 (P <0 0 1,P <0 0 5 )。结论　ET ,TXA2 含量及ET/ANP ,TXA2 /PGI2 比值升高与妊高征发病密切相关 ,比值增高幅度与病情严重程度成正比。Objective To study the changes of plasma endothelin(ET),thromboxin A 2(TXA 2),prostacyclin(PGI 2) and atrial natriuretic peptid(ANP) in pregnancy,and their relation to PIH.Methods The plasma ET,TXA 2,PGI 2 and ANP were measured four times respectively in different stage of pregnancy by radioimmunoassay.142 cases were normal pregnancy.36 cases were PIH.And 30 cases were taken as the control.Results ET decreased significantly in the middle stage of normal pregnancy(P<0 05),and there was no significant difference in the other 3 references between the normal pregnancy and the control group.In the late stage of pregnancy,the four references were significantly higher than those in the control group,but the ratios of ET/ANP and TXA 2/PGI 2 were almost the same.In the middle stage of PIH pregnancy,ET and TXA 2 were significantly higher than those in normal pregnancy(P<0 01,P<0 05).Moreover,the ratios of ET/ANP and TXA 2/PGI 2 increased significantly.In the late stage of PIH pregnancy,the 3 references in PIH were significantly higher than those in normal pregnancy except PGI 2(P<0 05,P<0 01).Conclusion ET,TXA 2 and the ratios of ET/ANP and TXA 2/PGI 2 are related to PIH.The higher they increase,the worse PIH is.
