摘 要:目的对国人离体肝脏进行门静脉空气造影,测量门静脉的分支数量和管径、门静脉右前叶分支数目、尾状叶门静脉分支数目及起始点与门静脉左右支交汇处的距离。方法离体肝脏解冻、负压吸引后,于门静脉内充入足够气体,行CT扫描并进行测量。结果 19个肝脏标本中,肝内门静脉正常型17个(17/19,89.47%),Ⅰ型变异2个(2/19,10.53%)。门静脉右支主干管径(9.41±1.46)cm,分支数量为(3.18±1.60)支;门静脉右前支管径(8.41±1.52)cm,分支数量为(8.94±3.03)支;门静脉右后支管径(6.94±1.98)cm,分支数量为(7.69±2.87)支;门静脉左支管径(10.95±2.25)cm,分支数量为(10.68±3.24)支。17个右前叶分为腹侧支及背侧支,2个分为右前上支和右前下支。腹背侧分支的腹侧支数目为7~9支,背侧支数目为7~11支。尾状叶门静脉的分支数目为1~3支。结论离体肝脏门静脉空气造影CT扫描可以准确显示肝内门静脉的分支类型、数量、管径、起始点位置。Objective To explore the diameter and the number of main branch of intrahepatic portal vein,and the number of right anterior branch of portal vein,the number of portal branches of caudate lobe,and the distance between the initial point of portal branches of caudate lobe and the intersection of left/right branch of the portal vein.Methods In vitro livers were filled with enough gas in the portal vein after thaw and vacuum aspiration,and CT scanning and measurement were done.Results Among 19 liver specimens,normal intrahepatic portal vein was found in 17(17/19,89.47%),while typeⅠ heteromorphosis of intrahepatic portal vein was found in 2(2/19,10.53%).The diameter and number of main branch of right portal vein was(9.41±1.46)cm and 3.18±1.60,of the right anterior branch of portal vein was(8.41±1.52)cm and 8.94±3.03,of the posterior branch of right portal vein was(6.94±1.98)cm and 7.69±2.87,of the branch of left portal vein was(10.95±2.25)cm and 10.68±3.24,respectively.The right anterior lobe were divided into ventral branch and the dorsal branch in 17 liver specimens,while in the other 2 liver specimens were divided into right anterior superior branch and right anterior inferior branch.The number of the ventral branches ranged from 7 to 9,the number of the dorsal branches ranged from 7 to 11,and the number of portal branches of caudate lobe ranged from 1 to 3.Conclusion In vitro CT air venography of liver specimens can accurately display the branch type,the number,the lumen diameter and the initial point of intrahepatic portal vein.
