外周血干细胞动员在治疗下肢缺血性疾病的应用及围术期护理

摘　　要：目的研究综合护理对外周血干细胞动员、自体外周血干细胞移植治疗下肢缺血性疾病的干预效果。方法选取94例下肢缺血性疾病患者,随机分为实验组和对照组,各47例,两组均接受外周血干细胞治疗,对照组使用常规护理,实验组在对照组的基础上进行综合护理,观察两组患者治疗效果及不良反应发生情况。结果治疗后两组患者静息痛和冷感发生率均显著降低,实验组静息痛发生率(27.9%)和冷感发生率(36.1%)均显著低于对照组的46.7%和55.0%(P<0.05)。移植后两组患者间歇性跛行症状均有较显著改善,实验组有效率为80.9%,对照组有效率为61.7%,实验组明显高于对照组(P<0.05)。治疗后实验组踝肱指数(ABI)为(6.74±2.72),心踝血管指数(CAVI)为(0.85±0.23),均显著高于对照组[(5.67±2.43)、(0.70±0.24)](P<0.05)。实验组不良反应发生率(4.2%)显著低于对照组(12.8%),差异有统计学意义(P<0.05)。结论外周血干细胞动员对下肢缺血性疾病有较好的治疗效果,而综合护理能有效提高患者移植成功率,降低术后不良反应发生率,改善治疗效果。Objective To study the intervention effect of synthetic nursing on peripheral blood stem cell therapy ap plied in lower limb ischemic disease.Methods 94 lower ischemia patients were divided into experimental group and control group randomly,with 47 cases in each group.Both groups accepted peripheral blood stem cell therapy and the control group was treated with norm nursing,while the experimental group was treated with synthetic nursing.The ef fectiveness of therapy and adverse reactions were observed.Results After treatment,the rates of rest pain and limb cold in both groups were all decreased.The experimental group had lower rates of rest pain（27.9%） and limb cold（36.1%） than control group（46.7%,55.0%）（P 0.05）.Experimental group had 80.9% improved in aspect of intermitted claudication than control group of 61.7%（P 0.05）.The CAVI and ABI of both groups increased significantly after therapy,while the experimental group had higher ABI of（6.74 ±2.72） and CAVI of（0.85 ±0.23） than control group [（5.67±2.43）,（0.70±0.24）]（P 0.05）.The experimental group had a lower adverse reaction rate of 4.2% than control group of 12.8%（P 0.05）.Conclusion Peripheral blood stem cell mobilization and transplantation is effective for lower limb ischemic disease and the synthetic nursing can improve the success rate of transplanting,reduce adverse reaction rate and improve therapeutic effect.
