慢性乙型肝炎患者外周血T淋巴细胞亚群变化临床研究

摘　　要：目的监测外周血T淋巴细胞亚群中CD3+、CD4+及CD8+细胞在慢性乙型肝炎患者和健康人群间的变化,研究慢性乙型肝炎患者在细胞免疫方面的动态。方法用流式细胞仪监测外周血CD3+、CD4+及CD8+细胞数量变化,用全自动生化分析仪检测血清中丙氨酸氨基转移酶(ALT)。结果慢性乙型肝炎患者外周血CD3+、CD4+细胞数及CD4+/CD8+比值明显低于健康人群(P<0.01),而CD8+细胞数明显高于健康人群(P<0.01)。慢性乙型肝炎患者中血清转氨酶异常者外周血CD3+、CD8+细胞数明显升高(P<0.01);CD4+细胞数略有升高,但无统计学意义,CD4+/CD8+比值明显下降(P<0.05)。结论慢性乙型肝炎患者T淋巴细胞的改变,可导致细胞免疫功能改变,使得乙肝病毒难以清除;慢性乙型肝炎患者CD8+细胞数与血清转氨酶浓度有平行关系。Objective The aim of the study is to monitor the changes of peripheral blood lymphocyte subsets, CD3＋ ,CD4＋ and CD8＋ between chronic hepatitis B patients and healthy people,and further to investigate the dynamic variations of cellular immunities of hepatitis B patients. Methods Flow cytometry （FACS） to count the changes of numbers of CD3＋ ,CD4＋ and CD8＋ lymphocytes,and the concentrations of alanine transaminase （ALT） in serum were detected by automatic biochemistry analyzer. Results The numbers of CD3＋ ,CD4＋ lymphocytes and CD4＊/CD8＋ ra- tio values of chronic hepatitis B patients were significantly lower than the healthy people （P-~0.01）. However, the CD8＋ cell numbers of chronic hepatitis B patients were much higher compared to the healthy people （P^0. 01）. Ill chronic hepatitis B patients with abnormal ALT, the numbers of CD3＋ and CD8＋ cells were remarkably increased （P~ 0.01）, and CD4＋ lymphocytes increased lightly （with no significance）. However, the CD4＋/CD8＋ ratio values of these patients decreased remarkably. Conclusion The changes of numbers and ratio values of T- lymphocytes will induce the cellular immunities changes, which further improve hepatitis B viruses＇ activities. Furthermore, there is positive correlation between the counts of CD8＋ lymphocytes of hepatitis B patients and the concentration of alanine transaminase.
