摘 要:目的探讨白细胞介素18(IL-18)基因编码区第105位点基因多态性与草酸钙肾结石病的关系。方法选取2010年6月至2011年10月在深圳市第二人民医院泌尿外科就诊的60例尿路结石患者列为草酸钙结石组,将同期来医院健康查体的志愿者60例作为对照组,用聚合酶链反应-限制性片段长度多态性方法检测所有研究对象的IL-18基因编码区第105位点基因型。结果经过TaqⅠ酶切的IL-18 DNA片段可分为三种基因型:AA(148 bp)、AC(148、123、25 bp)及CC(123、25 bp),草酸钙结石组AC+CC基因型(46.6%,28/60)与对照组(21.6%,13/60)相比有统计学意义(P<0.01),等位基因C在草酸钙结石组(25%,30/120)和对照组(10.8%,13/120)频率分布相比有统计学意义(P<0.01)。结论 IL-18基因编码区第105位点基因型多态性与草酸钙肾结石病有相关性,IL-18基因编码区第105位点C等位基因可作为肾石病患者草酸钙结石危险因素的标志。Objective To explore the relationship between interleukin-18(IL-18) gene 105 genetic polymorphism and calcium oxalate nephrolithiasis.Methods Sixty patients with calcium oxalate stones who were diagnosed in Shenzhen Second People's Hospital from June 2010 to October 2011 and a control group of 60 healthy subjects were selected.Interleukin-18(IL-18) gene 105 genotypes in the coding region were analyzed by polymerase chain reaction-restriction fragment length polymorphism.Results After digestion with TaqⅠ,IL-18 gene 105 genetic polymorphism was assumed to cause three genotypes according to the banding types as AA(148 bp),AC(148,123,25 bp) and CC(123,25 bp).The prevalence of the A/C + C/C genotypes(46.6%,28 /60) in the patients was higher than that in the controls(21.6%,13 /60,P 0.01).The allelic C frequency of IL-18 + 105A/C differed significantly between the patients and the controls(25% vs 10.8%,P 0.01).Conclusion IL-18 gene 105 genetic polymorphism appears to be associated with the calcium oxalate nephrolithiasis,and the IL-18 gene + 105 C allel may be a good marker for evaluating the genetic risks in patients with nephrolithiasis.
