瓜蒌延胡汤联合西医常规治疗卧位性心绞痛38例临床观察

摘　　要：目的观察瓜蒌延胡汤联合西医常规治疗卧位性心绞痛的临床疗效。方法将卧位性心绞痛患者64例随机分为2组。对照组26例予β受体阻滞剂、钙拮抗剂及硝酸脂类等治疗。治疗组38例在对照组治疗基础上加用瓜蒌延胡汤治疗。2组均15d为1个疗程,1个疗程后统计疗效,并观察2组治疗前后总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FPG)及超敏C反应蛋白(hs-CRP),全血黏度、血浆黏度、红细胞比容、血小板聚集率及纤维蛋白原,心率、收缩压(SBP)、舒张压(DBP)、左室舒张末期内径(LVDd)、左室射血分数(LVEF)及二尖瓣E峰与A峰比值(E/A)变化情况。结果治疗组心绞痛总有效率84.2%,对照组心绞痛总有效率46.2%,2组比较差异有统计学意义(P<0.05),治疗组疗效优于对照组;治疗组心电图总有效率60.5%,对照组心电图总有效率34.6%,2组比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。治疗组治疗后TC、TG、HDL-C、LDL-C、FPG及hs-CRP均降低(P<0.05),且治疗组治疗后低于对照组(P<0.05)。治疗组治疗后全血黏度、血浆黏度、红细胞比容、血小板聚集率及纤维蛋白原均降低(P<0.05),且治疗组治疗后低于对照组(P<0.05)。治疗组治疗后心率、SBP、DBP、LVDd、LVEF及E/A均有明显改善(P<0.05),且优于对照组(P<0.05)。结论瓜蒌延胡汤联合西医常规治疗卧位性心绞痛疗效确切。Objective To explore the clinical effect of Gualou Yanhu Decoction with conventional western medicine therapy on patients with decubitus angina.Methods 64 patients with decubitus angina were randomly divided into two groups including treatment group（n = 26） and control group（n = 38）.Patients in control group received β-blockers,calcium antagonists and nitric acid lipid.Patients in treatment group received Gualou Yanhu decoction on the basis of control group treatment.The course was 15 days in two groups.The changes of TC,TG,HLD-C,LDL-C,hs-CRP,blood viscosity,plasma viscosity,haematocrit,platelet aggregation and fibrinogen,heart rate,systolic blood pressure（SBP）,diastolic blood pressure（DBP）,left ventricular end diastolic diameter（LVDd）,left ventricular ejection fraction（LVEF） and mitral E and A wave ratio（E /A） before and after treatment were observed in two groups.Results The total effective rate of angina in treatment group（84.2%） was superior to that in control group（46.2%,P 〈0.05）.The total effective rate of ECG in treatment group（60.5%） was superior to that in control group（34.6%,P 〈0.05）.TC,TG,HDL-C,LDL-C,FBG and hs-CRP after treatment were decreased in treatment group（P 〈0.05）.Those in treatment group were decreased as compared with those in control group after treatment（P 〈0.05）.Blood viscosity,plasma viscosity,haematocrit,platelet aggregation and fibrinogen after treatment were decreased in treatment group（P 〈0.05）.Those in treatment group were decreased as compared with those in control group after treatment（P 〈0.05）.Heart rate,SBP,LVDd,LVEF and E/A after treatment were significantly improved in treatment group（P 〈0.05）.Conclusion Gualou Yanhu decoction combined with conventional western medicine therapy has definite effect on the treatment of patients with decubitus angina.
