摘　　要：目的总结胃癌手术中脾损伤的预防及处理方法。方法回顾性分析2007年7月至2012年12月一组胃癌手术发生脾损伤的临床资料。结果 586例胃癌手术中发生脾损伤29例(4.95%),其中Ⅰ级、Ⅱ级和Ⅲ级损伤分别为23例、5例和1例。保脾成功26例,全脾切除3例。全部病例治愈。结论提高医师对脾损伤的认识、创造术野良好显露、减少脾脏牵拉、谨慎且规范操作等是预防胃癌手术脾损伤的有效方法。多数的脾损伤采用伤处电凝后涂医用胶、脾裂伤缝合修补、脾动脉结扎等方法可满意控制出血。Objective To explore the prevention and treatment of spleen injury induced by gastric cancer operation. Methods The clinical data of splenic injury during gastric cancer operation occurred in July 2007 - December 2012 were analyzed retrospectively. Results 586 eases of surgery of gastric cancer occurred in patients 29 cases of splenic injury, splenic injury rate of 4.95%, including Ⅰ grade 23 ca- ses, Ⅱ grade 5 cases and Ⅲgrade 1 case. Spleen - preserving successful in 26 cases ,full - splenectomy in 3 cases. All patients recovered. Conclusion Improving the understanding of splenic injury in operation, creating a good exposure, reducing the spleen traction, at the same time, prudential and standard operation were effective ways to prevent splenic injury during gastric cancer operation. For majority of the splenic injury, by the wound coated with medical adhesive after electrocoagulation, or splenic repaired by suturing, or splenic artery ligated and other methods can be satisfied control bleeding.
