摘  要:目的通过对苯妥英钠血药浓度监测结果的分析,了解其血药浓度与临床疗效的关系。方法使用高效液相色谱法测定291名癫痫患者的血清中苯妥英钠的血药浓度,色谱柱选用ZORBAX SB-C18(4.6 nm×250 nm,5μm);流动相:磷酸二氢钾缓冲液(pH=3.5)-甲醛(39∶61)(V/V);检测波长:220 nm。在血清加入5μL硫酸溶液混匀后,使用重蒸乙醚萃取苯妥英钠。结果血药浓度在10.0~20.0μg/mL之间的有137例(47.08%),治疗总有效率为55.33%。结论苯妥英钠的个体动力学参数差异大,用药期间应定期监测血药浓度并结合患者表现来调整用药剂量,做到个体化给药方案,以提高药物疗效及确保患者用药安全。Objective To establish HPLC method to determine the concentration of Phenytoin sodium in serum, and to find out the best and effective serum concentration of Phenytoin sodium for patients with epilepsy. Methods The serum concentration of Phenytoin sodium was detected in 291 epileptic patients by HPLC. The chromatographic column was ZORBAX SB - C18 (4.6 mm × 250mam, 5 μm), the mobile phase was Potassium dihydrogen phosphate (pH = 3.5) and methanol (39:61 ), the flow rate was 1 ml_,/min, and the detective wavelength was 220 nm. 5 μL sulphuric acid solution (6M) was added to serum and was mixed evenly, then Phenytoin sodium was extracted by evaporated ethylether. Results The best effective serum level of 137 well controlled cases was 10.0 - 20.0 μg/mL. At the end of the treatment group the total effective rate was 55.33%. Conclusion The relationship between dosage taken and its efficacy varies in individuals. A close monitoring of serum concentration of phenytoin sodium should be conducted when needed.
