摘 要:目的探讨大肠埃希菌的耐药性特点和产超广谱β-内酰胺酶(ESBLs)菌株的基因型分布。方法对临床分离的128株大肠埃希菌进行耐药性统计和产ESBLs菌基因型分析。结果产ESBLs菌株对临床常用抗菌药物的耐药率远高于非产ESBLs菌株;产ESBLs菌株对氨苄西林、氨曲南和头孢噻肟的耐药率分别为85.7%(48/56)、89.3%(50/56)和96.5%(54/56);而非产ESBLs菌株对氨苄西林、氨曲南和头孢噻肟的耐药率分别为68.1%(49/72)、69.4%(50/72)及76.4%(55/72);产ESBLs菌株CTX-M型、TEM型及SHV型基因的阳性率分别为85.7%(48/56)、8.9%(5/56)、5.4%(3/56)。结论产ESBLs菌株在临床分离的大肠埃希菌中已较为普遍,且耐药率高,临床应根据药敏及耐药基因合理使用抗菌药物。Objective To explore the drug resistance characteristics of Escherichia coli(E.coli) and the genotype distri bution of extended-spectrum β-lactamases(ESBLs) producing E.coli.Methods 128 clinical isolates of E.coli were performed the statistical analysis on the drug resistance and the ESBLs genotype analysis.Results The resistance rates of ESBLs-producing isolates to common antibacterial drugs were far higher than that of non-ESBLs-producing isolates;the drug resistance rates of ESBLs-producing isolates to ampicillin,aztrenam and cefotaxime were all above 85.0%;while the resistance rates of non-ESBLs-producing isolates to ampicillin,aztrenam and cefotaxime were 68.1%,69.4% and 76.4% respectively.The positive rates of ESBLsproducing genotypes like CTX-M,TEM and SHV were 85.7%(48/56),8.9%(5/56) and 5.4%(3/56) respectively.Conclusion The isolates of ESBLs-producing E.coli is relatively common among the clinical isolates of E.coli,their drug resistance rate is high.Antibacterial drugs should be rationally used according to the drug susceptibility testing results and drug resistance genes.
