产后出血的相关高危因素和流行病学特征分析

摘　　要：目的:探讨产后出血的相关高危因素、流行病学特征,为进行有效的防治措施提供理论依据。方法:回顾性分析2002年1月～2011年6月在我院产科住院分娩6 059人临床资料,其中产后出血共223例,采用多因素分析产后出血的高危因素。结果:产后出血发生率3.68%,多因素Logistic回归分析结果显示分娩方式、多胎妊娠、子宫瘢痕、胎盘因素、巨大儿、产次是产后出血发生的相关危险因素(P<0.05),而高龄、孕次与产后出血发生无统计学意义(P>0.05)。分娩方式、多胎妊娠、子宫瘢痕、内科疾病、胎盘因素亦是产后出血量≥1 500mL的高危因素。结论:产后出血是产科最常见最危急的并发症之一,加强孕产妇系统管理,降低剖宫产率,规范治疗,对降低产后出血及孕产妇死亡起着非常重要的作用。Objective：To explore the risk factors and epidemiological characteristics of postpartum hemorrhage,and to provides the basic theory for the effective prevention and control measures.Methods： Data of 6059pregnancy cases including a total of 223cases of postpartum hemorrhage that were hospitalized in our hospital from January 2002to June 2011were retrospectively analyzed using multivariate analysis to study the risk factors of postpartum hemorrhage.Results：The incidence of postpartum hemorrhage was 3.68%,multivariate（Logistic regression）analysis showed that the mode of delivery,multiple pregnancy,uterine scar,placental factors,macrosomia,delivery times are related risk factors of postpartum hemorrhage（P0.05）,while the being elderly,times of pregnancy and occurrence of postpartum hemorrhage was not statistically significant factors（P0.05）.The mode of delivery,multiple pregnancy,uterine scar,involvement of internal disease,and placenta factor as well as postpartum hemorrhage amount which is greater than 1 500mL were all risk factors.Conclusion：Postpartum hemorrhage is one of the most common and most serious obstetric complications,strengthening the maternal system management to reduce the cesarean section rate,and standardized treatments are very important to prevention of decrease incidence of postpartum hemorrhage and maternal death.
