摘 要:目的:探讨不同对比剂流率对肝门静脉(Portal vein,PV)CT血管造影质量的影响,为优化门静脉CT血管成像技术提供临床依据。方法:选择2010年7月~2012年7月期间我院收治的80例接受肝门静脉多层螺旋CT上腹检查的患者为研究对象,按数字表法随机分为三组,分别为27例、27例、26例。前臂静脉注射对比剂碘比醇,浓度为300mgI/mL,A组流量为2.0mL/s,B组3.0mL/s,C组4.0mL/s。采用对比剂自动跟踪激发扫描技术进行扫描,分析比较三组的成像质量。结果:A组、B组、C组随着对比剂流量的增加,造影图像质量逐渐提高,且三组比较具有显著性差异(χ2=12.979,P<0.05)。其中,C组造影质量评价得分情况明显优于A组(χ2=11.334,P<0.05);而C组与B组相比,B组与A组相比无显著性差异(P>0.05)。A组、B组、C组随着对比剂流量的增加,PVCT值、肝实质CT值、PV-肝实质CT值均逐渐增加。三组的PVCT值、PV-肝实质CT值比较具有显著性差异(t=6.989,P<0.05),而肝实质CT值比较无显著性差异(P>0.05)。C组的PVCT值、PV-肝实质差明显高于A组(t=10.612、9.745,P<0.05);而C组与B组相比,B组与A组相比无显著性差异(P>0.05)。结论:对比剂注射流率对于患者门静脉CT血管造影图像质量有显著的影响,以4.0mL/s的对比剂注射流率得出的CT图像质量最佳。Objective : To study influence of flow velocity of contrast media on CT portography,and to provide clinical basis for optimization of CT portography.Methods : A total of 80cases scheduled for CT portography from July 2010to July 2012in our hospital were collected as the research object and randomly divided into group A(27cases),group B(27cases) and groupC(26cases).Contrast medium was injected through antebrachium veins with power injector at the rates of 2.0 mL / s(group A),3.0 mL / s(group B) and 4.0mL / s(group C).Image quality was analyzed and compared.Results : With increase of contrast material flow rate,image quality gradually improved.And the comparison between the three groups had significant difference(χ 2 =12.979,P0.05).Image quality of group C was obviously better than that of group A(χ 2 =11.334,P0.05).But between group C and group B or group B and group A,there was no significant difference(P0.05).With increase of contrast material flow rate,CT attenuation of hepatic portal venous trunk,liver parenchyma and PV-CT values gradually increased.CT attenuation of hepatic portal venous trunk and PV-CT values between the three groups had significant difference(t= 6.989,P0.05).But CT attenuation of liver parenchyma had no significant difference(P0.05).CT attenuation of hepatic portal venous trunk and PV-CT values of group C were obviously higher than them of group A(t=10.612 、 9.745,P0.05).But between group C and group B or group B and group A,there was no significant difference(P0.05).Conclusion : Contrast medium injection flow rate has a significant influence on CT portography.CT portography quality is the best with flow velocity of contrast media at 4.0mL/s.
