摘  要:目的:探讨丹参注射液对胸腔镜心脏手术围手术期的肺保护作用及其机制。方法:选取我院2006年2月~2012年11月收治的需行胸腔镜心脏手术的患者60例,随机分为观察组与对照组,观察组在常规治疗的基础上加用丹参注射液静脉滴注,对照组只采用常规治疗方法,对比两组患者肺泡-动脉血氧分压差(P(A-a)DO2)及呼吸指数(RI),并留取血清测定细胞间黏附分子-1(ICAM-1)浓度。结果:两组患者P(A-a)DO2、RI及ICAM-1在主动脉开放后均升高(P<0.01),观察组上述指标较明显低于对照组明(P<0.01);两组患者T0期时ICAM-1浓度对比无明显差异,T1、T2其时,ICAM-1浓度较T0期显著升高。结论:在胸腔镜心脏手术前后应用丹参注射液静脉滴注,对患者具有一定的肺保护作用。Objective : To study the pulmonary protective effect of salvia miltiorrhiza injection for thoracoscope heart surgery during perioperative period and its mechanism.Methods : A total of 60patients admitted in our hospital for thoracoscope cardiac surgery from 2006.2to 2012.11were randomly divided into trial group and control group.The control group was treated by conventional method,while the trial group was also given salvia miltiorrhiza injection through intravenous drip based on conventional method.Compared alveolar-arterial blood oxygen partial pressure difference(P(A-A) DO2),respiratory index(RI),and concentration of serum intercellular adhesion molecule 1(ICAM 1) between the two groups.Results : P(A-A) DO2,RI and ICAM-1were increased in two groups after aorta were opened(P 0.01),but the above mentioned parameter levels of the trial group were significant lower than that of the control group(P0.01);ICAM-1concentrations have no obvious difference between the two groups during T0.At T1,T2,while a significant rise in concentration of ICAM-1was observed.Conclusion : Salvia miltiorrhiza injection through intravenous drip has pulmonary protective effect during thoracoscope open-heart surgery.
