婴儿过敏性胃肠炎的临床表现、诊治及相关文献复习

摘　　要：目的总结临床表现以胃肠道表现为主的食物过敏的临床特点及诊治经验。方法回顾性收集中山市博爱医院确诊为过敏性胃肠炎的3例患儿,总结其临床表现,胃镜下表现,诊断,治疗,及随访情况。结果 2例患儿以进食后哭闹,呕吐,并呕吐咖啡样液体为主要表现,1例以腹泻为主要表现,3例均出现不同程度的体重不增,营养不良,贫血等情况,3例患儿胃镜检查显示胃粘膜糜烂,充血,损害。经过更换特殊奶粉喂养后,3例患儿病情均明显改善,无再呕吐,腹泻,贫血纠正。结论食物过敏可导致患儿呕吐,腹泻,消化道出血,等胃肠炎表现,同时可出现贫血,生长发育迟缓等。胃镜检查可见胃粘膜充血,糜烂,损害。食物过敏性胃肠炎无异性临床表现,诊断主要依靠病史及食物激发试验,回避试验,避免过敏原为最好的治疗方法。Objective The purpose of this study is to summarize the clinical manifestations,diagnosis and treatment experiences of food allergic gastroenteritis. Methods Summarize The clinical datas of 3 cases with food allergic gastroenteritis that were collected from Boai Hospital,Zhongshan City,where we diagnosed these diseases and made a successful treatment,including present history,presentation under Gastroscopy,diagnosis,treatment,and follow-up. Results The children of the two cases cried after eating,vomited,sometimes vomit coffee-like liquid,One case complained diarrhea as the main performance,Three cases present varied degrees underweight,malnutrition,anemia etc. All cases of gastroscopy examination showed that those children were suffered form gastric mucosal erosion,hyperemia. After replacing their food with special formula food,three sick children were improved significantly,vomiting,diarrhea were disappeared,and anemia corrected,gastroscopy examination show gastric mucosal normal. Conclusion Food allergy could lead children to vomiting,diarrhea,gastrointestinal hemorrhage,gastroenteritis,Also could lead to anemia,growth retardation, and so on. Gastroscopy examination showed gastric mucosal hyperemia,erosion. Food allergic gastroenteritis has no special manifestations. The diagnosis rely on history and food challenge test and food-avoid testing. According to the analysises that were made,we have a conclusion that avoiding food allergens is the best method of treatment for food allergic gastroenteritis.
