35 Cases of Moderate and Severe Neonatal Hypoxic Ischemic Encephalopathy Treated with Integrated Chinese and Western Medicine

摘　　要：目的探讨复方丹参、神经节苷脂(ganglioside,GM1)和高压氧(hyperbaric oxygen,HBO)在新生儿缺氧缺血性脑病(hyoxicischemic encephalopathy,HIE)中的疗效。方法按照HIE诊断及分度标准,排除宫内感染、遗传代谢性疾病及其他先天性疾病所引起的脑损伤基础上,将172例中、重度HIE患儿随机分为治疗组和对照组,予以常规治疗。治疗组则在此基础上分别加用GM1、HBO、GM1和HBO、GM1和HBO加用复方丹参注射液,观察和记录临床恢复体征、恢复情况和20项新生儿行为神经测定(neonatal behavioral neurological assessment,NBNA)评分。结果 HBO治疗组和GM1治疗组的临床疗效和NABA评分与对照组相比,差异无统计学意义;但GM1联用HBO组及"活血化瘀"中西医结合治疗组和对照相比,疗效优于对照组,且差异有统计学意义。结论复方丹参、GM1和HBO对中、重度HIE患儿有改善临床症状、缩短病程,提高治愈率的作用。Objective To explore the efficacy on neonatal hypoxic ischemic encephalopathy（HIE） treated with compound salvia miltiorrhiza, ganglioside （GM1）and hyperbaric oxygen （HBO）. Methods In reference to HIE diagnosis and degree standard, excluding brain damage induced by intrauterine infection, ge netic metabolic diseases and other congenital disease, 172 moderate and severe children HIE were randomized into a treatment group and a control group. They were treated conventionally. In the treatment group, on the basic conventional treatment, GM1, HBO and GM1, and HBO, GM1 and HBO combined with compound salvia mihiorrhiza injection were applied separately. The clinical recovery of physical signs, recovery condition and 20 -item neonatal behavioral neurological assessment（NBNA） score were observed and recorded. Results Compared with the control group, the clinical efficacy and NABA score were not significantly different in HBO treatment group and GM1 treatment group. The efficacy was different significantly in GM1 plus HBO group and the group of integrated Chinese and western medicine for activating blood circulation and removing stasis as compared with the control group. Conclusion Compound salvia miltiorrhiza,GM1 and HBO in combina- tion relieve the clinical symptoms, shorten the duration of sickness and improve the curative rate in moderate and severe neonatal HIE.
