摘  要:目的探讨一蒂双叶胫后动脉穿支皮瓣修复肢体皮肤软组织缺损的方法及临床疗效。方法 2006 ̄05—2012 ̄05依据胫后动脉各穿支分布的解剖学特点,设计并切取以胫后动脉为蒂的双叶穿支皮瓣修复肢体两邻近创面16例,将两分叶穿支皮瓣重新组合改形后修复不规则创面6例。其中游离移植12例,同侧带蒂转移8例,健侧桥式转移2例。结果 22例44叶皮瓣均完全成活,随访时间6个月至3年,皮瓣外形满意,质地优良,供区植皮无溃疡或磨损。结论对于肢体两邻近创面、洞穿性创面以及不规则形状的创面,采用胫后动脉分叶穿支皮瓣进行修复,具有设计灵活、手术操作相对简单,皮瓣供血可靠等特点,具有较高的临床应用价值。Objective To explore the method and clinical effect of repairing skin and soft tissue defects with monopedicled bilobed posterior tibial artery perforator flap. Methods From May 2006 to May 2012 according to the anatomic characteristics of distributions of all perforating branches of the posterior tibial artery, bilobed perforator flaps pedicled with posterior tibial artery were designed to be cut out to repair adjacent wounds on the limb (16 cases), and two lobulated perforator flaps were recombined and reshaped to repair irregularly shaped wounds (6 cases), including free grafting in 12 cases, homolateral pedicled transfer in 8 cases and contralateral bridge-induced translocation in 2 cases. Results The 44-lobed flaps of 22 cases survived completely. The follow- up of 6 months to 3 years revealed satisfactory appearance of flap with fine texture and no anabrotic or abrasive skin graft at the donor sites. Conclusion Limb's adjacent wounds, piercing wounds and irregularly shaped wounds could be repaired with posterior tibia artery perforator flaps, which are characterized by design flexibility, relatively simple surgical oPeration and reliable blood-supply for flaps, and are of higher clinical application value in repairing complicated wounds mentioned above.
