摘  要:目的 观察硝酸甘油和艾司洛尔对围拔管期心血管反应的影响。方法  6 0例ASAⅠ~Ⅱ全麻病人随机分为三组 ,于术毕符合气管拔管指征时 ,Ⅰ组静注艾司洛尔 0 .5mg·kg-1,Ⅱ组静注硝酸甘油 10 μg·kg-1,Ⅲ组静注生理盐水 3ml,观察围拔管期血压、心率 ,并计算血压心率乘积 (RPP)。结果 Ⅲ组病人围拔管期血压、心率及RPP升高 (P <0 .0 5 ) ,Ⅰ组血液动力学趋平稳 ,Ⅱ组血压变化不明显 ,而心率及RPP则于拔管时及拔管后一分钟显著增高。结论 艾司洛尔比硝酸甘油更能有效预防围拔管期心血管反应。Objective To explore the effect of esmolol and nitroglycerin on cardiovascular responses during tracheal extubation period. Methods 60 patients under going general anesthesia(ASA Ⅰ~Ⅱ) were randomly divided into 3 groups (20 in each group). When had indications of extubation, patients in groupⅠ received 0.5mg/kg esmolol, 10μg/kg nitroglycerin for group Ⅱ and physiological saline 3ml for group Ⅲ. SBP、DBP、HR、RPP were recorded before extubation, during removing secretions, during extubation and 1、5、10 min after extubation, respectively. Results No significantly hemodynamic changes occurred in group Ⅰ.In group Ⅱ, SBP、DBP kept stable while HR and RPP increased markedly during and 1 min after extubation . Compared with basic level before administration, SBP、DBP、HR and RPP in group Ⅲ increased significantly during tracheal extubation period. Conclusion Esmolol is more effective for attenuating cardiovascular responses than nitroglycerin druing tracheal extubation period.
