急诊PCI术中心室电风暴10例分析

摘　　要：目的总结急诊经皮冠状动脉介入治疗（PCI）术中心室电风暴的治疗、抢救方法、注意事项及转归。方法对10例急诊PCI术中心室电风暴患者的资料进行回顾性分析。结果10例均在急诊PCI术中发生电风暴，从手术开始到电风暴时间最短为8min，最长为20min，平均（12．00±3．71）min，患者均在术中给予电除颤和药物治疗，7例为非同步直流电击除颤，共植入支架13枚。2例术中植入IABP辅助治疗，1例术后植入IABP辅助治疗，2例安装心脏临时起搏器，10例均未出现死亡。结论急诊PCI术中心室电风暴患者应及时分析病因，给予电复律，配合药物治疗，必要时植入IABP或心脏临时起搏器，尽快完成PCI术恢复冠脉血液灌注，可成功终止心室电风暴。Objective To summarize the emergency percutaneous coronary intervention （PCI） intraoperative ventricular electrical storm treatment, rescue methods, matters needing attention and prognosis. Methods The present study retrospectively analyzed 10 cases of intraoperative ventricular electrical storm during emergency PCI operation. Results These ten cases all happened electrical storm during emergency PCI operation, The time period from the beginning of operation to the occurrence of electrical storm was from 8 to 20 min, and averaged of（ 12.00± 3.71 ） min. The patients were given electrical defibrillation and drug therapy in the operation, 7 patients were treated with non synchronous DC defibrillation, 13 stents were implanted altogether. 2 patients was implanted IABP during operation, 1 cases was implanted IABP after operation, 2 patients was installed temporary cardiac pacing, 10 patients were not death. Conclusion When intraoperative ventricular electrical storm happens during emergency PCI operation, the causes should be analyzed at the first time. Administration of cardioversion combined with drug treatment, if necessary, implantaion of temporary cardiac pacemaker or IABP were propor therapeutic measures. Complete the PCI operation and restore coronary blood perfusion as soon as possible help to terminate ventricular electrical storm effectively.
