摘  要:目的:观察重度烧伤休克患者早期炎症因子C反应蛋白(CRP)、白细胞介素1(IL-1)和细胞黏附分子1(ICAM-1)的改变。方法:自2011年1月1日—2012年12月31日,选取沈阳市重度烧伤休克患者35例,设为烧伤组;同期体检健康者36例,设为对照组。烧伤组患者于入院时(T0)、入院后12 h(T1)、24 h(T2)、48 h(T3)各时点抽取静脉血,测定血清CRP、IL-1和ICAM-1的含量。对照组于体检后相应时点抽取静脉血检测CRP、IL-1和ICAM-1的含量。结果:烧伤组和对照组比较,T0时CRP含量明显增多(P<0.01),T1时CRP和IL-1含量均明显增多(均P<0.01),T2、T3时,CRP、IL-1和ICAM-1含量均明显增多(均P<0.01);烧伤组患者血清CRP、IL-1和ICAM-1含量均随时间呈上升趋势,各指标各时间点间差别均具有统计学意义(均P<0.01),对照组各指标各时间点间差别均无有统计学意义(均P>0.05)。结论:重度烧伤休克患者早期炎性反应发挥重要作用,这为指导临床用药提供了理论依据。Objective:To investigate the early changes in inflammatory cytokines of patients with extensive burn during the shock period. Methods:Thirty- five patients with burn shock were included in the study as burn group since January 1,2011 to December 31,2012. Thirty - six healthy subjects were chosen as control group. The inflammatory cytokines CRP, IL - 1, and ICAM - 1 in serum of the patients were detected after 0 hour,12 hours,24 hours,and 48 hours admitted to the hospitals. At the corresponding time point after the medical examination, levels of CRP, IL - 1, and ICAM - 1 in the healthy were detected. Results : The serum levels of CRP, IL - 1 and ICAM - 1 were significantly higher in burn group than those in control group at 24 hours and 48 hours after admitted to the hospitals (all P 〈0. 01 ). In the burn group,the serum levels of CRP,IL - 1 and ICAM - 1 were significantly increased since 0 hour to 48 hours after admission ( all P 〈0. 01 ). In the control group,there was no statistically sig- nificant difference between the time points of each index ( all P 〉 0. 05 ). Conclusion : The extensive burn patients have apparent inflammatory response immediately after trauma during the shock stage.
