狼疮肾炎患者血FIB、D-D、FDP水平变化及其与肾损害的关系

摘  要:目的:了解狼疮肾炎(LN)患者纤溶系统变化及其与肾损伤的关系。方法:对48例LN和45例健康人群的血浆D-二聚体(D-D)、纤维蛋白原(FIB)、纤维蛋白降解产物(FDP)及24 h尿蛋白定量进行检测并进行相关分析。结果:(1)LN组血浆D-D、FDP、FIB均明显高于正常对照(P<0.01);(2)LN组D-D、FDP、FIB与24 h尿蛋白定量呈显著正相关(r=0.514,P<0.01;r=0.388,P<0.01;r=0.321,P<0.05)。结论:LN患者存在凝血、纤溶异常,继发性纤溶可能参与肾损伤的过程。Objective : To investigate the changes of fibrinolysis system in patients with lupus nephritis (LN) and the relation- ship with kidney injury. Methods : Plasma fibrinogen ( FIB), D - dimer ( D - D ), fibrin degradation products (FDP) and 24 - hour urinary protein excretion were investigated and the correlation was analyzed in 48 patients with LN and 45 healthy controls. Re- sults : ( 1 ) The plasma FDP, FIB and D - D level in the LN group were significantly higher than those in the control group ( P 〈 0. 01 ). (2)There was significantly positive correlation between the levels of plasma D -D, FDP, FIB and 24 -hour urinary pro- tein excretion( r = 0. 514 ,P 〈 0. 01 ;r = 0. 388, P 〈 0. 01 ;r = 0. 321, P 〈 O. 05 ). Conclusion:There were the abnormality of blood coagulation and fibrinolysis in LN. The secondary fibrinolysis is probably involved in the process of kidney injury in LN patients.
