摘 要:目的 探讨咳喘落协同地塞米松拮抗支气管哮喘气道变应性炎症作用的分子机制。方法 采用逆转录 聚合酶链反应 (RT PCR)半定量地测定了不同剂量的咳喘落对哮喘大鼠肺组织和外周血单个核细胞(PBMC)中IL 4mRNA、IL 5mRNA表达的影响。结果 哮喘时大鼠肺组织和PBMC中IL 4mRNA与IL 5mRNA表达均增强 ,显然高于空白对照组 (P <0 .0 1) ,经大剂量咳喘落治疗后 ,其表达同时显著降低 (P <0 .0 5 ,P <0 .0 1) ,与地塞米松组无差异 (P >0 .0 5 ) ,小剂量咳喘落治疗对哮喘大鼠肺组织中IL 4mRNA的表达有显著的抑制作用 (P <0 .0 5 ) ,但小剂量咳喘落加地塞米松对哮喘大鼠肺组织和PBMC中IL 4mRNA、IL 5mRNA表达的抑制显著强于大剂量咳喘落和地塞米松 (P <0 .0 5 ,P <0 .0 1)。结论 咳喘落能够同时降低大鼠肺组织和PBMC中IL 4和IL 5基因转录 ,减少其生物合成 ,进而抑制嗜酸性粒细胞的聚集、活化 ,发挥其抗气道炎症的作用 。Objective To investigate the molecular mechanism of synergistic effect of dexamethasone and KeChuanLuo on allergic airway inflammation. Method The expression levels of interleukin 4,5(IL 4,5) mRNA in lung tissues and peripheral blood mononuclear cells (PBMC) in asthmatic rats treated with different dosages of KeChuanLuo were examined semi quantitatively by reverse transcription polymerase chain reaction (RT PCR). Results The levels of the expression of IL 4,5mRNA in both lung tissues and PBMC in the model group were higher than those in normal group ( P <0.01), but were significant decreased in higher dosage of KeChuanLuo treated group ( P <~0.05~ P <0.01), and no differences was seen between the higher dose KeChuanLuo treated group and dexamethason treated group ( P >0.05). Only significantly decrease of IL 4mRNA expression levels in lung tissues were found in low dose KeChuanLuo treated group ( P <0.05). The expression levels of interleukin 4,5 (IL 4,5) mRNA in both lung tissues and PBMC from groups treated with low dose KeChuanLuo and dexamethason were significantly less than that from groups with higher dose KeChuanLuo or dexamethason ( P <0.05~ P <0.01). Conclusion KeChuanLuo shows its effects on anti airway allergic inflammation by marked inhibition of the expression levels for IL 4,5mRNA in the lung tissue and in PBMC, reduction of synthesis of IL 4,5, inhibition of eosinophilic aggregation and activation in the airway. KeChuanLuo is synergistic with dexamethasone. (Shanghai Med J, 2000,23∶470 473)
