国产封堵器介入治疗室间隔缺损1002例分析

摘　　要：目的探讨与室间隔缺损介入治疗相关的并发症的发生率、发生原因及防治方法。方法观察随访2004年3月至2012年1月河北医科大学第一医院心内科收治的1002例室间隔缺损患者使用国产封堵器介入治疗术中和术后的并发症发生情况。结果介入治疗总成功率97．31％（975／1002），总并发症发生率13．97％（140／1002），其中心脏并发症124例，血管并发症11例、其他并发症5例。严重并发症发生率1．20％（12／1002），其中Ⅲ度房室传导阻滞5例，重度三尖瓣关闭不全3例，封堵器脱落1例，基底节区脑梗死1例，股动脉血栓2例，经相应治疗后无死亡。随访时间3—54个月，平均（28．3±25．9）个月。结论室间隔缺损介入治疗安全、有效，并发症发生率低。术前正确判断、术中准确操作和术后严格随访是减少并发症发生的有效措施。Objective To discuss the incidence, causes, prevention and management of complications associated with interventional therapy for ventrical septal defect（VSD） with domestic occluders. Methods One thousand and two patients with VSD were received interventional therapy. Intraoperative and postoperative complications were observed and followed up. All the patients were treated with domestic occluders. Results The total success rate of interventional therapy was 97.31% （975/1002） and the complication incidence was 13.97% （140/1002）. There were 124 cases of cardiac complication, 11 cases of vascular complications and 5 cases of other complications. The incidence of severe complications was 1.2% （ 12/ 1002） ,including 5 cases of third degree atrioventricular block,3 cases of severe tricuspid regurgitation, 1 case of occluder shedding, 1 case of basal ganglia infarction and 2 cases of femoral arterial thrombosis. None of them died from relevant treat- ment. The follow-up time was 3 - 54 （ averaged 28. 3± 25. 9 ） months. Conclusion Interventional therapy of VSD is safe and effective and the incidence of complication is low. Correct judgment before operation, accurate manipulation during operation and strict follow-up after operation are effective measures to decrease complications.
