摘　　要：目的调查和分析上海市三级甲等医院手术安全核查制度执行情况。方法随机抽取2011年度上海市五家三级甲等医院住院手术患者的病历10 000份,对病历内的手术安全核查表进行调查分析。结果在所调查的10 000份住院患者病历中,共存有手术安全核查表9 997份。每家医院的手术安全核查表都有涂改的现象,总体的涂改率为1.69%。五家医院手术安全核查表均存在填写不完整的问题,平均完整率为97.54%。结论上海市三级甲等医院手术安全核查制度执行基本情况令人满意,但仍有改进和提高的空间。Objective To analyze the implementation of surgical safety verification system in 5 tertiary hospitals in Shanghai area. Methods 10 000 medical records of hospitalized surgery patient were selected randomly from five tertiary hospitals in the year 2011 in Shanghai area, the surgical safety checklists within which were investigated and analyzed. Results There were 9 997 copies of surgical safety checklist in the 10 000 hospitalized patients medical records. Each hospital＇s surgical safety checklist has the phenomenon as alteration, with the overall altered rate of 1.69%. and incompleteness, with the average completeness rate of 97.54%. Conclusions The implementation of surgical safety verification system in tertiary hospitals in Shanghai area is fairly satisfactory, but there is still room for further improvement.
