摘  要:目的:研究雾化布地奈德或使用抗生素对无基础肺疾患的呼吸机相关性支气管炎(VAT)患者影响。方法:将无基础肺疾患的VAT患者171例随机分为抗生素组、雾化组和对照组,分别给予抗生素、布地德雾化治疗和生理盐水雾化治疗,治疗期7天,观察期3个月,比较各组呼吸机相关性肺炎(VAP)发生率、机通气时间、入住ICU时长及死亡率。结果:抗生素组、雾化组VAP发生率及死亡率均显著低于对照组,P<0.0三组的ICU住院日、呼吸机辅助通气时间均无明显差异(P>0.05)。结论:对无基础肺疾患的VAT患者,雾化入布地奈德或短期使用抗生素能降低其VAP发生率及死亡率。Objective: To study the effects of aerosol inhalation of Budesonide and using antibiotic to the patients with ventilator-associated tracheobronchitis (VAT). Methods: 171 patients with VAT were randomly divided into antibiotic group,aerosol inhalation group and control group and accepted antibiotic,aerosol inhalation of Budesonide and aerosol inhalation of NS for 7 days,compared the incidence rate of VAP,mortality rate,days of mechanical ventilation and average of ICU stay for 3 months. Results: The incidence of VAP and mortality in antibiotic group, aerosol inhalation group were significantly lower than those in the control group, though the days of mechanical ventilation and average of ICU stay were no statistically significant among the three groups (P 〉 0.05). Conclusion: Aerosol inhalation of Budesonide or short-term using of antibiotics can reduce the incidence rate and mortality of VAP for patients with no other pulmonary disease.
