血浆B型尿钠肽在老年心力衰竭中的临床意义

摘　　要：目的:探讨血浆B型尿钠肽(BNP)水平在老年心力衰竭(HF)诊疗中的临床意义。方法:将2010年8月-2012年1月我科临床诊断为心力衰竭并收治入院的124例老年患者为观察组,并选择同期我科收治入院的50例非心力衰竭患者作为对照组,比较两组患者BNP水平及左室射血分数(LVEF),比较观察组内超声心动图参数关系及不同病因所致同级心衰BNP水平。结果:(1)观察组BNP及LVEF水平与对照组存在显著性统计学差异(P<0.01),不同心功能NYHAⅠ、Ⅱ、Ⅲ和Ⅳ分级之间BNP及LVEF水平有显著性差异(P<0.05);(2)LVEF≤40%血浆BNP水平明显高于LVEF>40%者(P<0.01),左室舒张末期内径(LVEDD)≤55mm的血浆BNP明显低于LVEDD>55 mm者(P<0.01);(3)观察组BNP治疗后显著低于治疗(P<0.01)。结论:检测血浆BNP水平对老年HF的诊疗具有重要的临床意义。Objective： To investigate plasma B-type natriuretic peptide （BNP） level detection of clinical significance in elderly patient with heart failure （HF）. Methods： 124 elderly patients with I-IF were selected as the observation group and 50 patients with non-heart failure were selected as the control group from Aug 2010 to Jan 2012. The plasma BNP level and LVEF were compared between the two groups, and the BNP levels were compared according to echocardiographic parameters, different causes due to the HF in the observation group. Results： （1） There was significantly difference of plasm BNP and LVEF between the two groups（P〈0.01） and among NYHAⅠ, Ⅱ , Ⅲ and iV group （P〈0.05）. （2） The BNP level in the LVEF 〈~40% group was significantly higher than that in LVEF〉 40% group （P〈0.01）, The BNP level in the LVEDD≤55mm group was significantly lower than that in LVEDD〉55 mm group （P〈0.01）. （3） The BNP level in patient with HF was significantly lower after-treatment than that in pre- treatment （P〈0.01）. Conclusions： It has an important clinical significance that BNP level was detected in the diagnosis and treatment of elderly patient with HF.
