不同肠外营养在儿童腹型过敏性紫癜中的应用比较

摘　　要：目的:比较不同肠外营养(PN)在腹型过敏性紫癜(HSP)中的应用效果。方法:将本科收治的腹型HSP患儿42例随机分为对照组和观察组各21例,均给予禁食及常规治疗,对照组PN给予葡萄糖及小儿氨基酸静滴,观察组PN给予全营养混合液(TNA)静滴;于治疗前及治疗后7 d测静脉定视黄醇结合蛋白(RBP)、前白蛋白(PA)、转铁蛋白(TRF)、血清白蛋白(ALB)水平;治疗前及出院前测定IgA、C3、C4及超敏C反应蛋白(hsCRP)水平,测量其体重、记录住院天数,比较两组上述指标。结果:治疗后两组RBP、PA、TRF及体重增长值有显著差异(P<0.01),住院天数有明显差异(P<0.05);但ALB、IgA、C3、C4及hsCRP无明显差异(P>0.05)。结论:腹型HSP在禁食期间应用TNA支持治疗可改善营养状态,对机体的促炎作用及免疫功能影响不明显,减少患儿体重的消耗,利于患儿康复,缩短了住院天数。Objective：To compare application effects of different parenteral nutrition （PN） in treatment for henoch-schonlein purpura （HSP） of abdominal type. Methods：42 patients with HSP hospitalized in pediatrics were randomly divided into control group and observation group, each group was 21. Abrosia and conventional treatment were conducted for the two groups, glucose and pediatric amino acid were provided for the control group, and PN total nutrient admixture （TNA） was provided for the observation group through intravenous drip. Venous blood samples were collected before and after 7 days the treatment to measure the levels of retinol binding protein （RBP）, prealbumin （PA）, transferrin （TRF） and serum albumin （ALB）. The levels of IgA, C3, C4, high-sensitivity C-reactive protein（hsCRP） and the body masses were measured, and inpatient days were recorded before and after the treatment. The differences were compared between the two groups. Results：After the treatment, the two groups were compared with RBP, PA, TRF and value of body masses （P〈 0.01）, inpatient days （P〈 0.05）,ALB, IgA,C3,C4 and hsCRP （P 〉 0.05）. Conclusion： TNA support during abrosia period could improve nutritional status, decrease body mass consumption, effect no obvious on proinflammatory and immune function of organism, contribute to recovery of children with HSP of abdominal type and shorten inpatient time.
