B-Lynch缝合术在剖宫产术中难治性宫缩乏力性出血的应用

摘　　要：目的分析研究B-Lynch缝合术在剖宫产术中难治性宫缩乏力性出血中的应用效果。方法我院在2011年1月～2013年1月期间共接收剖宫产术中难治性宫缩乏力性出血患者20例,将其随机分成实验组和对照组,均在采用手法按摩、缩宫素、益母草针剂、前列腺素治疗无效后,对照组(10例)给予传统的宫腔填塞纱布术,实验组则使用B-Lynch缝合术治疗,对比分析两组的治疗效果及并发症情况。结果实验组有效10例,有效率为100%;对照组有效6例,有效率为60%,差异具有统计学意义(P<0.05)。实验组的手术时间、术中及术后24 h的出血量明显低于对照组,差异具有统计学意义(P<0.05)。实验组无并发症,对照组并发症发生率为30%,差异具有统计学意义(P<0.05)。结论 B-Lynch缝合术可明显提高剖宫产术中难治性宫缩乏力性出血的治疗有效率,并明显降低并发症的发生,方法更简单、更迅速,应在临床中推广应用。Objective To analyze the pplication effect of B-Lynch suture technique in intractable postpartum hemor- rhage caused by uterine inertia in cesarean section. Methods The 20 cases of patients with intractable postpartum hemorrhage caused by uterine inertia in cesarean section were divided into experimental group and control group with l0 cases in each. After treated by the massage technique, oxytocin, leonurus injection, prostaglandin with invalid, the control group was given traditional uterine tamponade gauze, experimental group was given B-Lynch suture surgery treatment, compared therapeutic effect and complications between the two groups. Results In the experimental group, 10 cases were effective, effective rate was 100%, in control group 6 cases was effective, effective rate was 60%, with significant differences （P 〈 0.05）. The operation time, amount of bleeding in intraoperative and postoperative 24 h in experimental group was lower than that of control group, with significant difference （P 〈 0.05）. And experimental group with no complications, in control group the incidence of complications was 30%, had significant difference （P 〈 0.05）. Conclusion B-Lynch suture technique can obviously increase the effective rate of treatment of intractable postpartum hemorrhage caused by uterine inertia in cesarean section, and obviously reduce the complications, the method is more simple, more quickly, so it should be applied in clinical.
