摘  要:目的探讨神经节苷脂对重型颅脑损伤患者血清神经元特异性烯醇化酶(NSE)水平的影响及疗效。方法70例重型颅脑损伤患者随机分为观察组和对照组,均予以对症支持治疗。观察组患者加用神经节苷脂静滴,连用2周。结果治疗2周后,两组患者血清NSE水平均有明显下降(P<0.05或P<0.01),且观察组下降的幅度较对照组更明显(P<0.05);观察组患者治疗2周后的GCS评分明显高于对照组(χ2=6.83,P<0.05)。观察组患者治疗后1个月内意识恢复清醒率明显高于对照组(χ2=4.79,P<0.05),清醒时间明显短于对照组(P<0.05)。观察组患者治疗后3个月GCS评分明显高于对照组(χ2=6.30,P<0.05)。两组患者治疗期间未见明显的药物不良反应。结论神经节苷脂能明显提高重型颅脑损伤患者的GCS评分,促进患者的意识恢复,缩短清醒时间,作用机制与其降低血清NSE水平密切相关。Objective To discuss ganglioside's influence and curative effect observation of serum neuron specific eno- lase (NSE) level in severe craniocerebral injury patients. Methods A total of 70 cases of severe craniocerebral injury patients were divided into observation group and control group at random. The patients in the two groups were both given symptomatic and supportive treatments. The patients in observation group shall be given ganglioside for 2 weeks. Results After 2 weeks' medical treatment, serum NSE levels of patients in both groups had obviously declined (t= 2.31,3.15 ,P 〈 0.05 or P 〈 0.01), and the decrease rate in observation group was much higher than that in control group (t=2.24,P 〈 0.05); the GCS score of patients in observation group was much higher than that in control group (X2=6.83 ,P 〈 0.05). After 1 month's medical treatment, the rate of recovery of consciousness of patients in observation group was much higher than that in control group (X2=4.79 ,P 〈 0.05); the time of recovery of consciousness of patients in observation group was much shorter than that in control group (t=2.31 ,P 〈 0.05). After 3 month's medical treatment, the GCS score of patients in observation group was much higher than that in the control group (X2=6.30,P 〈 0.05). No obvious adverse drug reaction appeared in both two groups. Conclusion Ganglioside may obviously improve the GCS score of severe craniocerebral injury patients, promote the recovery of consciousness of patients, shorten the time of re- covery of consciousness,, whose mechanism of action has close effect on reducing the levels of serum NSE.
