肱骨近端锁定钢板内固定在36例老年肱骨近端骨折患者中的临床疗效研究

摘　　要：目的比较肱骨近端锁定钢板及AO斜T形钢板内固定治疗老年肱骨近端骨折患者的临床疗效,旨在选择合适的固定器材。方法对我院收治的72例老年肱骨近端骨折患者随机分为A、B两组,每组36例,分别采用肱骨近端锁定钢板和AO斜T形钢板进行内固定治疗,并对两组的术中情况、肩关节评分及临床疗效进行对比分析。结果 B组术中的切口长度、手术时间、术中出血量显著低于A组,且术后B组的肩关节功能评分显著高于A组,差异具有统计学意义（P〈0.05）。对全部患者进行6~12个月的随访,A组的优良率为66.7%,B组的优良率为优良率为94.4%,两组差异具有统计学意义（χ2=8.87,P〈0.01）。结论肱骨近端锁定钢板及AO斜T形钢板内固定在老年肱骨近端骨折患者的治疗中均具有一定的疗效,但肱骨近端锁定钢板内固定在术中情况、术后肩关节功能及远期疗效上显著优于AO斜T形钢板内固定。Objective To select the appropriate fixed equipment by comparing the clinical efficacy of proximal humeral locking plate fixation and AO skew T-shaped plate fixation in the treatment of proximal humeral fractures. Methods 72 patients with proximal humeral fractures were divided into group A and B with 36 patients in each group, patients in group A and B received treatment of proximal humeral locking plate fixation and AO skew T-shaped plate fixation, intraoperative conditions, shoulder scores and clinical efficacy were analyzed and compared. Results Length of the incision, operative time and intraoperative blood loss of group B were lower than that in group A significantly, shoulder scores of group B were higher than group A significantly. The difference was statistically significant（P〈0.05）. All the patients were followed up 6 to 12 months,excellent rate of group A and B were 66.7% and 94.4% respectively, the difference of two groups was statistically significant（~=8.87, P〈0.01）. Conclusions Both proximal humeral locking plate fixation and AO skew T-shaped plate fixation have a certain efficacy in the treatment of proximal humeral fractures, while the efficacy of proximal humeral locking plate fixation were better than AO skew T-shaped plate fixation in intraoperative conditions, shoulder scores and long-term efficacy.
