摘  要:目的分析硫化氢(H2S)对严重烧伤大鼠盲肠部分泌性免疫球蛋白A(SIgA)及盲肠肠组织T淋巴细胞亚群CD3+、CD4+、CD8+淋巴细胞数以及CD4+/CD8+比值变化的影响,探讨H2S在烧伤后肠黏膜免疫屏障中的作用。方法将104只SD纯种雄性大鼠,体质量220~250g,随机分为烧伤组(96只)和正常对照组(8只),其中烧伤组随机分为单纯烧伤组(32只)、烧炔丙基甘氨酸(PPG)干预组(32只)、硫氢化钠(NaHS)干预组(32只)。正常对照组不烫伤,其余各组背部剃毛造成30%TBSAⅢ度烫伤后,各烧伤组分别于伤后2d、7d、14d及21d四个时相点取8只SD大鼠盲肠及内容物,用双G抗体-PEG放射免疫法测定SIgA;用流式细胞术(FCM)检测盲肠肠壁组织中T淋巴细胞CD3+、CD4+、CD8+数量及并计算CD4+/CD8+比值的变化。结果与正常对照组比较,烧伤组各时相点SIgA均降低;烧伤组各时相点CD3+、CD4+降低,CD8+明显升高,CD4+/CD8+降低,差异均有统计学意义(P<0.05);与单纯烧伤组比较,NaHS干预组SIgA水平在伤后各时相点均升高,CD3+、CD4+明显升高,CD4+/CD8+升高,差异均有统计学意义(P<0.05);与单纯烧伤组比较,PPG干预组各时相点SIgA水平降低,CD3+、CD4+明显降低,CD4+/CD8+降低,差异均有统计学意义(P<0.05)。结论外源性HS对严重烧伤大鼠的肠道免疫屏障有保护作用。Objective To analyse the effect of hydrogen sulfide (H2S) in severely burned rats on secretory IgA and the changes of T lymphocyte subsets CD3 + , CD4 + , CD8 + T cells and CD4 +/CD8 + , thus to investigate the effect of H2S on the intestinal mucosal immune barrier after burned. Methods One hundred and four SD purebred male rats weighing 220-250 g were randomly divided into burn groups (96 rats) and normal group (8 rats). Ninety-six rats of the burn groups were randomly divided into simple burn group, NariS group and PPG group (32 rats in each group). The normal group did not burn, and the rats backs of the other three groups caused 30% TBSA full-thickness scald after shaving. The cecum and intestinal contents of the burn groups were collected after feeding a week and at 2 d,7 d, 14 d and 21 d after burned, and the changes of SigA, CD3 +, CIM+, CD8 +T cells and CD4+/CD8+ were analyzed. Results Compared with the normal group, SigA levels of the burn groups were decreased in various phases, and CD3 + , CD4 + decreased, CD8 + increased, CD4 +/CD8 + decreased in the burn groups at each time point (each P 〈 0.05 ). compared with the simple bum group, SIgA levels of NariS group were elevated in all time points after injury, CD3 + , CD+ and CD4 +/CD8 + increased ( each P 〈 0.05). Compared with the simple burn group, the levels of SigA, CD3 + and CD8 + in PPG group decreased significantly at each time point, while CD4 +/CD8 + decreased (each P 〈 0.05). Conclusion Exogenous H2S has protective effect on intestinal immune barrier in severe burn rats.
