NT-proBNP对不同年龄组心源性呼吸困难和肺源性呼吸困难患者的鉴别意义

摘　　要：目的:分析测定NT-proBNP浓度值对不同年龄组的心源性呼吸困难和肺源性呼吸困难患者的鉴别诊断意义。方法:收集心源性呼吸困难患者76例和肺源性呼吸困难患者64例,根据不同年龄段NT-proBNP参考区间不同,将其分为三个年龄组,即50岁以下为一组,50～75岁为一组,75岁以上为一组,测定每个患者的NT-proBNP浓度值,对每个年龄组的心源性呼吸困难患者和肺源性呼吸困难患者的NT-proBNP数值进行比较。结果:50岁以下心源性呼吸困难患者NT-proBNP值(780±286)pg/ml,肺源性呼吸困难患者NT-proBNP值(140±39)pg/ml;50～75岁心源性呼吸困难患者NT-proBNP值(2 430±1 110)pg/ml,肺源性呼吸困难患者NT-proBNP值(260±99)pg/ml;75岁以上心源性呼吸困难患者NT-proBNP值(4 666±1 650)pg/ml,肺源性呼吸困难患者NT-proBNP值(400±178)pg/ml。三个年龄组的心源性呼吸困难患者和肺源性呼吸困难患者的NT-proBNP数值差异均有统计学意义(t=9.681,21.919,22.512,P<0.01)。结论:NT-proBNP在不同年龄组对于鉴别诊断心源性呼吸困难和肺源性呼吸困难都是有很好的临床意义的。Objective To ananlyze the significance of NT-proBNP in differential diagnosis of cardiac dyspnea or pulmonary dyspnea.Method From the year of 2009 to 2011,we collected 76 patients with cardiac dyspnea and 64 patients with pulmonary dyspnea.According to different age,different NT-proBNP reference range,they were diveded into three groups to measure the concentrations of serum NT-proBNP.Results In the below 50 years old group,between 50 to 75 years old group and more than 75 years old group,the cardiac dyspnea patients′concentrations of NT-proBNP were markedly higher than those of pulmonary dyspnea,respectively.（780±286、2 430±1 110、4 666±1 650;140±39、260±99、400±178）pg/ml（t=9.681,21.919,22.512,P＆lt;0.01,respectively）.Conclusion The NT-proBNP play a significant role in differential diagnosis of cardiac dyspnea or pulmonary dyspnea to different age groups.
