摘 要:目的研究邵阳市中心医院多重耐药鲍曼不动杆菌分子流行病学特征与相关耐药基因流行情况。方法对临床分离的40株多重耐药鲍曼不动杆菌经采用肠杆菌科间一致重复序列PCR方法同源性分析获得的A、B、C、D 4种克隆株,应用聚合酶链反应及序列分析方法分析耐药基因类型。结果本院多重耐药鲍曼不动杆菌共检测出TEM-1、OXA-23和PER-1三种耐药基因,未检出SHV、DHA、OXA-24、GES、IMP、VIM耐药基因。TEM-1检出率为100%,OXA-23(45%),PER-1(10%),其中2株菌同时检测到TEM-1、OXA-23和PER-1三种耐药基因,分别为A型和C型各1株,另2株菌同时检测到TEM-1和PER-1两种耐药基因,分别为C型和D型各1株,有16株菌同时检测到TEM-1和OXA-23两种耐药基因。结论携带TEM-1和OXA-23耐药基因多重耐药鲍曼不动杆菌是本院主要流行菌株,且存在克隆流行。携带TEM-1、OXA-23、PER-1三种耐药基因的菌株和携带TEM-1、PER-1两种耐药基因的菌株在临床分离株中已不鲜见。临床应加强鲍曼不动杆菌医院感染的控制,防止耐药菌株在医院暴发流行。Objective To study the molecular epidemiology and related drug resistance genes of multidrug resistant Acine- tobacter baumannii in Shaoyang Central hospital. Methods Forty isolates were grouped into four types of clones (type A, B, C and D) by enterobacterial repetitive intergenic consensus PCR (ERIC- PCR). Drug resistance genes were amplified and se- quenced. Results Three drug resistance genes, TEM- 1, OXA- 23 and PER- 1, were detected in the forty strains, and re- sistance genes SHV, DHA, OXA- 24, GES, IMP and VIM were not detected. The detection rates of TEM - 1, OXA - 23 and PER - 1 were 100 %, 45 % and 10 %, respectively. TEM - 1, OXA- 23 and PER - 1 were simultaneously detected in two iso- lates, one type A and one type C. TEM- 1 and PER- 1 were simultaneously detected in two strains, one type C and one type D. Sixteen isolates had both TEM - 1 and OXA- 23. Conclusions The multidmg resistant Acinetobacter baumannii with resistant genes TEM - 1 and OXA- 23 is the main prevalent strain in Shaoyang Central Hospital. Isolates carrying three drug - resistance genes, TEM- 1, OXA- 23 and PER- 1 and two genes, TEM- 1 and PER - 1, are not rare. Effective infection con- trol measures should be taken in order to prevent the outbreak of the drug resistant strains.
