摘 要:目的探讨高血压对颈动脉粥样硬化(AS)的影响以及血流灰阶显像(BFI)技术在动脉粥样硬化诊断中的应用价值。方法分别采用BFI技术和彩色多普勒血流显像(CDFI)技术检测198例高血压患者(高血压组)和200例正常血压者(对照组)的颈动脉内-中膜厚度(IMT)、粥样斑块检出率。分析血压与颈动脉粥样硬化斑块之间的关系。结果高血压组颈动脉IMT及粥样斑块检出率明显高于对照组,差异有统计学意义(P<0.01)。高血压组中IMT及粥样斑块检出率随高血压分级的增高而增高,组间比较差异有统计学意义(P<0.01或P<0.05)。BFI技术在斑块检出率上优于CDFI技术,两者比较差异有统计学意义(P<0.01)。结论高血压能加重颈动脉粥样硬化,且高血压的程度与动脉粥样硬化程度呈正相关。BFI技术对检测颈动脉粥样硬化有较高的使用价值。Objective To investigate the effect of hypertension on carotid atherosclerosis,and the clinical significance of BFI technique in the diagnosis of carotid atherosclerosis.Methods Using BFI technique and CDFI technique to detect 198 cases of patients with hypertension(hypertension groups) and 200 cases with normal blood pressure(control group) of carotid internal-media thickness(IMT) and the number of atherosclerotic plaque.To explore the relationship between hypertension and carotid atherosclerosis.Results IMT and the detection rate of plaque of hypertension groups was significantly higher than which of control group(P0.01).In hypertension groups,the higher of blood pressure levels,the higher carotid IMT and the detection rate of plaque(P0.01 or P0.05).The detection rate of plaque in BFI technique was higher than which in CDFI technique(P0.01).Conclusion hypertension can aggravate carotid atherosclerosis,and the higher blood pressure levels,the more obvious of carotid atherosclerosis.BFI technique is useful for the detection of carotid atherosclerosis.
