改良PPH术治疗老年急性嵌顿痔的综合效果观察

摘　　要：目的探讨改良PPH术治疗老年急性嵌顿痔的临床价值及优越性。方法将113例老年急性嵌顿痔患者分为观察者(67例)和对照组(46例),对照组采用外剥内扎术治疗,观察者采用改良PPH术治疗,观察两组患者治疗疗效。结果观察组手术时间、术中出血量、创面愈合时间及住院时间均明显低于对照组(P<0.05);观察组术后24 h及72 h疼痛VAS评分均明显低于对照组(P<0.05);观察组术后肛门坠胀、尿潴留及水肿的发生率均明显低于对照组(P<0.05)。结论改良PPH术治疗老年急性嵌顿痔具有创伤小、恢复快、疼痛轻、并发症少等优点,且手术操作简单,值得临床推广应用。Objective To investigate the clinical values and superiority of modified PPH operation on the treatment of aged acute incarcerated hemorrhoid.Methods 113 cases of aged acute incarcerated hemorrhoid were divided into the observation group（67 cases） and the control group（46 cases）.The control group was treated with Milligan-Morgan operation,while the observation group was treated with modified PPH operation.Observations were carried out about the treatment efficacy of both groups.Results As for the observation group,its operation time,intra-operative bleeding amount,healing time of the wound and the hospitalization time were all significantly lower than those of the control group（P 0.05）.And its VAS ratings of pains 24 hours and 72 hours after the surgery were both significantly lower than those of the control group（P 0.05）.Its occurrence of post-operative anorectal ache,uroschesis and edema was significantly lower than that of the control group（P 0.05）.Conclusion With the advantages of small wounds,quick recovery,few pains and complications and simple operations,modified PPH operation is worth clinical application in the treatment of aged acute incarcerated hemorrhoid.
