摘　　要：目的比较优势手与非优势手留置PICC实施握力锻炼的效果。方法选择入院需要留置PICC进行化疗的150例肿瘤患者,随机分为观察组与对照组,观察组在非优势手置管,对照组在优势手置管,两组患者置管后均使用握力器锻炼,在置管皮肤外涂喜辽妥软膏,观察两组患者机械性静脉炎、导管异位、导管相关性感染等并发症的发生率和患者的疼痛评分情况。结果两组患者置管后1周内机械性静脉炎、导管异位、导管相关性感染差异无统计学意义(P>0.05);置管后1～5 d疼痛评分差异有统计学意义,观察组疼痛较轻(P<0.01)。结论 PICC在非优势手置管配合握力锻炼,不增加置管相关并发症,患者疼痛感减轻,值得推广应用。Objective To compare the effect of grip exercise in dominant and nondominant hand and to discuss complications and pain score of indwelling PICC in non-dominant hand.Methods Altogether 150 cancer inpatients undergoing chemotherapy by PICC were randomly divided into observation group and control group.In observation group,PICC was placed in non-dominant hand while in control group in dominant hand.Then patients in two groups clenched the hand-muscle developer and Mucopolysaccharide Polysulfate Cream was applied externally around the placement.The incidence of complications such as mechanical phlebitis,catheter ectopic,catheter related infections as well as patients’ comfort and pain score were recorded and observed.Results Within one week after catheterization,the incidence of mechanical phlebitis,catheter ectopic,catheter related infections was not statistically significant in two groups（P〈0.05） but 1 to 5 days after cathetering pain score indicated statistical significance（P〈0.01）.Conclusion It is better to indwell PICC in non-dominant hand and with grip exercise,which can effectively reduce local pain after inserting and even avoid the complications of PICC indwelling.
