子宫动脉下注射甲氨蝶呤并栓塞治疗剖宫产术后瘢痕妊娠的疗效分析

摘　　要：目的探讨子宫动脉下注射甲氨蝶呤并栓塞治疗剖宫产术后瘢痕妊娠的疗效。方法回顾性分析2012年3月至2013年1月在我院治疗的58例瘢痕妊娠患者,随机分为试验组和对照组,每组29例,试验组经双侧子宫动脉注射甲氨蝶呤并栓塞治疗,对照组给予药物治疗,治疗时间为1周,分析患者在治疗前后的孕囊大小、血清人绒毛膜促性腺激素(β-HCG)、并发症等情况,同时对患者进行随访1年,观察疗效。结果 2组孕囊大小在治疗前后比较,差异有统计学意义(t=0.56,t=4.01;P<0.05);而各组孕囊大小在治疗前后比较,差异有统计学意义(t=7.84,t=5.00;P<0.05);血清β-HCG 2组在治疗前后比较,差异有统计学意义(t=1.60,t=10.65;P<0.05);各组血清β-HCG在治疗前后比较,差异有统计学意义(t=157.43,t=157.54;P<0.05)。结论子宫动脉下注射甲氨蝶呤并栓塞对剖宫产术后瘢痕妊娠的效果较好,能有效减小孕囊大小,利于终止妊娠。Objective To study the curative effect of injection of methotrexate and embolism from uterine artery to treat cesarean section scar pregnancy. Methods A retrospective analysis of 58 cases scar pregnancy pa- tients treated in our hospital, they were randomly divided into experimental group and control group, each group had 29 people, the experimental group was given bilateral uterine artery injection of methotrexate and embolization treatment, control group was given drug treatment, the treatment time was a week, analysis size of pregnant bur- sa, serum beta HCG before and after treatment, complications, as well as the patients were followed up 6 months, observation curative effect. Results After different treatments, pregnant bursa size of two groups pa-tients in comparison before and after treatment, respectively, t= 0.56, t= 4.01; P〈0.05; And pregnant bursa size of each groups in comparison, respectively, t=7.84, t=5.00; P〈0. 05; serum Beta HCG compared in two groups, respectively, t=l. 60, t=10.65; P〈0. 05;beta HCG in serum of each group in comparison, respective-ly, t=157.43, t=157.54 ; P〈0.05. Conclusion The effect of injection of methotrexate and embolism from uter-ine artery to treat cesarean section scar pregnancywas good, can effectively reduce the pregnant bursa size, condu- cive to terminate a pregnancy.
