慢性乙型肝炎患者5种细胞因子水平的检测及其意义

摘　　要：目的探讨病毒性乙型肝炎患者mIL 2R ,sIL 2R ,IL 6 ,IL 8和TNF α与乙肝发病的关系。方法采用A PAAP技术和ELISA法 ,检测92例慢性乙型肝炎患者mIL 2R ,sIL 2R ,IL 6 ,IL 8和TNF α的水平。结果慢性乙肝患者mIL 2R的表达显著低于正常对照组(P<0.05) ,在PHA激活后与正常人基本一致 ,但仍较PHA激活前显著增高(P<0.01) ;慢性乙肝患者血清sIL 2R ,IL 6 ,IL 8和TNF α明显高于正常对照组(P<0.01)。且在慢性活动性与非活动性肝炎之间 ,肝硬化活动期与稳定期之间 ,重型肝炎的肝坏死期与恢复期之间 ,血清TNF α ,IL 6和IL 8 ,3项指标的差异显著(均P<0.001)。结论病毒性肝炎患者血清细胞因子的高低 ,可在一定程度上反映肝细胞损害的程度 ,也提示上述细胞因子参与了乙型病毒性肝炎的发病过程。Aim To study the relationship between the levels of mIL 2R, sIL 2R, IL 6, IL 8 and TNF αand pathogenesis in patients with chronic hepatitis B. Methods Levels of four serum cytokines(IL 6, IL 8, TFN αand sIL 2R) and expression of mIL 2R were detected by sandwich ELISA and APAAP respectively in 92 cases of chronic hepatitis B. Results The expression of mIL 2R in the patients was significantly lower than that in the control group(P<0.05) and its expression increased to the level of control group after PHA stimulation, but the mIL 2R expressions of the patients and normal controls after PHA stimulation were markedly higher than those before stimulation(P<0.01). The levels of serum sIL 2R, IL 6, IL 8 and TNF αin the patients with chronic hepatitis B were significantly higher than those in normal group(P<0.01). There was a great difference(P<0.01) in the levels of TNF α, IL 6 and IL 8 among patients with chronic hepatitis or liver cirrhosis at active and stable stages, and patients with severe type hepatitis at necrosis and recovery stages. Conclusion The level or expression of abovementioned cytokines reflected liver damage to some degree, and suggested that they involve in the pathogenetic process of hepatitis B.
