PPH治疗重度痔病术后并发症原因分析及处理

摘　　要：为探讨吻合器痔上黏膜环切术（PPH）治疗Ⅲ、Ⅳ期痔病术后常见并发症的发生率、可能原因，并提出处理措施。对应用PPH治疗的Ⅲ、Ⅳ期痔病385例进行术后观察，记录各种并发症发生的例数，并对术后患者的下腹疼痛程度、尿潴留、术后出血情况、肛门疼痛和排便情况进行评价，针对性地进行个体化处理。结果显示，下腹疼痛167例（43．4％），术后出血32例（8．3％），术后尿潴留128例（33．2％），肛门疼痛57例（14．8％），排便困难25例（6．5％），经及时有效的处理，患者均治愈出院。结果表明，应用PPH治疗Ⅲ、Ⅳ期痔病，并发症少，症状轻，恢复快，住院时间短，但对术后并发症仍需认真观察、及时处理。In order to investigate the incidence,possible causes of postoperative complication of Ⅲ, Ⅳ stage hemorrhoids treated with PPH, and to put forward their management measures, authors postoperatively observed the 385 patients with Ⅲ, Ⅳstage hemorrhoids subjected to PPH, recorded the incidence case number of various complications, and evaluated the relevant status of lower abdominal pain, uroschesis, postoperative bleeding, anal pain and defecation, as well as took individuality management measures accord- ing to these complications. As results,these complications included lower abdominal pain in 167 cases（43. 4 % ）, postoperative bleeding in 32 cases （8.3% ）, postoperative uroschesis in 128 cases （33.2%）, pain of anus in 57 cases（14.8%） ,and dyschesia in 25 cases（6.5%） ,on which timely and effective management was performed,so that patients were cured and discharged. Results show that though PPH in the treatment of Ⅲ, Ⅳ stage hemorrhoids has advantages such as less complication, mild symptoms, faster recovery and shorter hospitalization time,etc,but after operation it still is necessary seriously to observe,and in time to manage complication.
