摘  要:目的:探讨治疗浓度的雷诺嗪对人心房肌细胞电活动的影响。方法:从冠状动脉旁路移植术患者右心耳分离心房肌细胞,在治疗浓度的雷诺嗪作用下,使用膜片钳技术记录动作电位(APD)及峰钠电流、L型钙通道电流(LCa,L)、瞬时外向钾电流(Ito)、超快速延迟整流钾电流(IKur),观察给药前后的变化。结果:10μmol/L雷诺嗪使APD50和APD90分别延长27.5%和14.2%,而对静息电位和动作电位的幅度无明显影响;使钠电流失活曲线V0.5从(-92.9±7.8)mV左移至(-103.5±3.9)mV,对ICa,L、Ito、IKur的电流-电压曲线无明显影响。结论:10μmol/L雷诺嗪延长APD50和APD90,并使快钠通道失活增加。Objective To investigate the effect of ranolazine at therapeutic concentration on the electrical activity of human atrial myocytes. Methods Atrial myocytes were isolated from the right atrial appendage in patients with coronary artery bypass grafting. The patch-clamp technique was used to record action potentials (APD) and sodium peak current, L-type calcium channel current (ICa,L), transient outward potassium current (Ito), ultra- rapid delayed rectifier potassium current (IKur) before and after administration with ranolazine. Results The duration of the action potential at APDso and APD90 was increased 27.5% and 14.2% by 10 μmol/L ranolazine. Ranolazine had no effect on resting membrane potential or action potential amplitude. The steady state inactivation curve of INa was significantly shifted to the left. The V0.s for inactivation was (-103.5 ± 3.9 ) mV for the control and (-92.9 ± 7.8 ) mV for uses of ranolazine. Ranolazine had no effect on ICa,L, IKur or Ito. Conclusions 10 μmol/L ranolazine prolongs the action potential duration in human atrial myocytes and increases INa inactivation.
