常熟市1973～2010年居民主要恶性肿瘤减寿分析

摘　　要：目的对1973～2010年常熟市居民主要恶性肿瘤死亡情况进行减寿分析,为制定恶性肿瘤防控措施提供依据。方法收集死因报告责任单位上报的死因资料,按ICD-9、ICD-10的标准和方法进行分类,分析38年来主要恶性肿瘤减寿年数、减寿率、标化减寿年数、标化减寿率、平均减寿年数情况。结果 1973～2010年导致常熟市居民减寿的主要恶性肿瘤依次为胃癌、肝癌、肺癌,男性居民主要恶性肿瘤的减寿年数是女性居民的1.86倍,减寿率是女性居民的1.88倍;不同时期主要恶性肿瘤标化减寿率呈下降趋势,但肺癌和乳腺癌表现为不降反升,且减寿率所占比重增加,胃癌标化减寿率在各时期均居首位,肺癌标化减寿率位次上升;白血病平均减寿年数始终居不同时期首位,女性宫颈癌、乳腺癌平均减寿年数表现为上升趋势。结论恶性肿瘤是导致常熟市居民减寿的重要原因,应根据不同人群、不同肿瘤的特点,采取综合性干预措施,预防和减少恶性肿瘤对居民的寿命损失。OBJECTIVE To analyze the life lost due to malignant tumors in residents of Changshu from 1973 to 2010, so as to provide basis for prevention and control measures of malignant tumors. METHODS Death data were collected from duty units for report and classified by ICD-9 and ICD-10. Analyzed years of potential life lost （PYLL）, years of potential life lost rate （PYLLR）, standardized years of potential life lost （SPYLL）, standardized years of potential life lost rate （SPYLLR）, average years of life lost （AYLL） of major malignant tumors in 38 years. RESULTS From 1973 to 2010, the main malignant tumors causing resident’s life lost were gastric cancer, liver cancer and lung cancer in turn.Sex ratio of the PYLL of male to female was 1.86：1, and that of PYLLR was 1.88：1. The SPYLL of main malignant tumors had being gradually decreased in different periods, but that in lung cancer and breast cancer increased consistently, and the PYLL proportion of them increased in different periods. The SPYLL of gastric cancer was all the first, and the place of SPYLL of lung cancer was gradually rising. The AYLL of leukaemia was the first in different periods, and that of cervical cancer and breast cancer in females was increasing consistently. CONCLUSION Malignant tumors is the main cause of life lost which threatens the residents in Changshu.Comprehensive intervention measures should be taken to prevent and decrease the life lost of malignant tumors according to the different population and tumors.
