淮安市2009～2011年手足口病病原学监测结果分析

摘　　要：目的分析淮安市2009～2011年手足口病病原学监测结果,了解手足口病病原构成,为制定防治对策提供科学依据。方法使用SPSS13.0对国家疾病信息监测报告管理系统的个案资料进行统计分析,对部分病例进行病原体检测。结果 2009～2011年淮安市手足口病共报告病例15512例,0～4岁年龄组报告14171例。共抽样检测病原1237例,阳性689例,检测阳性率55.70%。2009年检出CA16占阳性病例的71.05%,2010和2011年检出EV71分别占阳性病例的60.00%、42.58%。200例重症病例中,EV71检出率35.50%;1037例普通病例中,EV71检出率24.40%,差异有统计学意义。92例检测出病原体的重症病例中,EV71检出率77.17%。结论 CA16、EV71为淮安市手足口病主要病原病毒,趋于以EV71感染为主;重症病例的EV71检出率高于普通病例,EV71感染易引起重症并发症;0～4岁儿童是防治的重点人群,重症病例防治的重点是EV71感染的防治。OBJECTIVE To investigate pathogen monitoring results of hand-foot-mouth disease （HFMD） in Huai’ an from 2009 to 2011, so as to understand the pathogen distribution of HFMD and provide scientific suggestions for making prevention strategy. METHODS SPSS13.0 was used to analyze the occurrence of HFMD cases base on National Disease Supervision Information Management System, and some of these cases were underwent pathogen detection. RESULTS A total number of 15512 HFMD cases were reported in Huai’an, 2009-2011, in which 14171 cases were at 0-4 age group. In 1 237 sampled cases underwent pathogen detection, 689 （55.70%） cases were pathogen positive. For all pathogen positive cases, CA16-positive rate in 2009, EV71-positive rate in 2010 and 2011 were 71.05% , 60.00% and 42.58%, respectively. EV71-positive rate of 200 severe cases was 35.50% , significantly higher than that of 1037 normal cases （24.40%）. Additionally, the detection rate of EV71 was 77.17% in 92 severe cases with pathogen-positive. CONCLUSION CA16, especially EV17, are the main pathogens of HFMD in Huai’ an. The detection rate of EV17 in severe group is much higher than that in normal ones, indicating that EV17 infection could induce severe complications. Children under 5-year-old are susceptible population should be focused on. Preventing EV17 infection is the key point to control severe cases of HFMD.
